高分级动脉瘤显微夹闭手术时机对患者预后的影响

The Timing of Operation on the Prognosis of Patients with High Grade Aneurysm Microscopy

目的:探讨高分级颅内动脉瘤显微手术夹闭时机对患者预后转归的影响。方法:回顾性分析2012年1月至2017年5月在我院神经外科经过显微夹闭手术治疗的80例Hunt-HessⅢ~Ⅳ级颅内动脉瘤患者的临床资料,按照患者发病时间到手术时间,分为早期40例(0~3d)、晚期40例(≥8d)两个亚组,统计两组手术患者的一般资料、可能与术后转归相关的因素、术后并发症及两组术后6个月转归情况。结果:早期组与晚期组在Fisher分级、术后迟性缺血性神经功能障碍、转归良好方面,差异有统计学意义(P<0.05)。结论:高分级颅内动脉瘤患者预后转归,早期手术优于晚期手术。

Objective： To investigate the effect of microsurgical clipping time on prognosis of patients with high grade intracranial aneurysms. Methods： The clinical data of 80 patients with Hunt-Hess III ~ IV intracranial aneurysms treated by microclipping in our department of neurosurgery from January 2012 to May2017 were retrospectively analyzed. According to the onset time of patients with operation time,divided into 40 cases of early（0 ~ 3）,40 cases of late（more than 8） two subgroups. The general data of two groups of surgical patients,factors associated with postoperative outcomes,postoperative complications and the outcome of the two groups after 6 months of operation were observed. Results： In the early group and the late group,the difference was statistically significant（P 〈 0.05） in the Fisher classification,the delayed ischemic nerve dysfunction and the good outcome. Conclusion： The prognosis of the patients with high grade intracranial aneurysm with early operation is better than that of late operation.
