高频超声测量视神经鞘直径在预测颅内压增高中的应用价值 被引量:10

摘要 目的探讨高频超声测量视神经鞘直径(ONSD)在预测颅内压(ICP)增高中的应用价值。方法收集2013-10—2016-12在郑州大学第二附属医院神经内科就诊,需要进行腰椎穿刺的患者127例,根据脑脊液压力的测量结果将患者分为颅内压增高组和颅内压正常组,比较2组患者的ONSD。结果颅内压增高组和颅内压正常组的ONSD差异有统计学意义;以ONSD≥5.07mm为诊断ICP增高的最佳阈值,灵敏度为86.5%(45/52)、特异度为86.7%(65/75),阳性预测值为81.8%(45/55),阴性预测值为90.3%(65/72)。结论高频超声测量ONSD对预测ICP增高有较高的临床应用价值。 Objective To evaluate the value of ultrasonographic optic nerve sheath diameter for assessing intracranial pres- sure. Methods 127 cases from October 2013 to December 2016 were enrolled in this study. The patients were graded to two groups according to the measurement results of CSF pressure. Results ONSD of two groups were significantly different. Taking ONSD≥5.07 mm as the efficacy evaluation, the sensitivity, specificity, positive predictive value, negative predictive value were 86.5% (45/52),86.7% (65/75),81.8%(45/55),and 90.3% (65/72) , respectively. Conclusion The high frequency ultrasound measurement of ONSD has high clinical value in predicting the increasing of ICP.
