摘要
目的:研究植入式静脉输液港与经外周静脉穿刺中心静脉置管(PICC)在恶性肿瘤患者化疗中的效果。方法:本次研究选取的研究对象为2015年3月1日-2016年2月28日在笔者所在医院进行治疗的恶性肿瘤患者,将88例患者计算机随机分为两组,每组44例。其中,一组患者实施PICC导管化疗(对照组),另一组采用植入式静脉输液港化疗(观察组)。将两组恶性肿瘤患者的置管情况、生活质量评分及并发症发生情况进行比较。结果:观察组患者4、8、12个月的导管置管率95.45%、81.82%、68.18%,意外拔管率4.55%,导管留置时间(205.65±75.84)d,均优于对照组,差异均有统计学意义(X^2=4.06、4.53、4.63、9.72,t=8.01,P<0.05);生活质量评分(86.88±10.96)分,导管堵塞、导管尖端异位、导管渗漏及血流感染的并发症发生率6.82%、2.27%、4.55%、9.09%,均优于对照组,差异均有统计学意义(t=10.26,X^2=14.07、9.65、8.49、9.32,P<0.05)。结论:在恶性肿瘤患者化疗中采用植入式静脉输液港的效果明显较PICC导管更优越,具有较高的有效性和安全性。
Objective:To study the effect of implantable venous port with PICC catheter in the chemotherapy of malignant tumor.Method:The research object of this study selected from March 1 st 2015 to February 28 th 2016 of patients with malignant tumor treated in our hospital,88 cases of patients were randomly divided into 2 groups by computer,44 cases in each group.The implementation of PICC catheter chemotherapy group(control group),another group with implantable venous port chemotherapy(observation group).Two groups of patients with malignant tumor of catheterization,quality of life score and complications were compared.Result:The observation group of patients with malignant tumor of the catheter catheterization rate with 95.45%,81.82%,68.18% in 4,8,12 months,the accidental extubation rate of 4.55% and catheter indwelling time of(205.65±75.84)d were better than those of the control group,the differences were statistically significant( X^2=4.06,4.53,4.63,9.72,t=8.01,P0.05).The quality of life score(86.88±10.96) points,the complication rates of catheter blockage in 6.82%,ectopic catheter tip in 2.27%,conduit leakage in 4.55% and bloodstream infection in 9.09% were better than those of the control group,the differences were statistically significant(P0.05).Conclusion:The implanted venous port is obviously better than PICC catheter in chemotherapy of patients with malignant tumors,and it is safe and effective.
