丘脑底核电刺激治疗帕金森非运动症状的疗效分析 被引量:3

摘要 目的分析丘脑底核电刺激术(STN-DBS)对于帕金森非运动症状的改善效果。方法对2014~2016年就诊于我院行STN-DBS的25例患者进行前瞻性分析,分析患者术前及术后开机1周时的相关评分。结果25例患者中,MDS-UPDRS-Ⅲ评分在术后下降26.5分,存在疲劳感的患者21例(84%),平均VAS-f得分在术后显著降低,且在术后三个月时未复发;存在焦虑的患者20例(80%),平均STAI得分在术后有降低,但差异没有统计学意义;存在疼痛相关困扰的患者15例(60%),平均VAS-P得分在术后显著降低,且疼痛症状在术后三个月时未复发;存在直立位头晕的患者22例(88%),但直立位低血压无改善。结论 STN-DBS能够改善帕金森病的运动症状和部分非运动症状,但仍需长期随访以评估其对不同患者的疗效。 Objective To study the effectiveness of subthalamic deep brain stimulation on non--motor syndromes in Parkinson's disease. Methods 25 patients whom underwent surgical treatment in our center during 2014--2016 were studied retrospectively. Results There was im- provement in 25 cases underwent STN-- DBS on MDS-- UPDRS-- III scale after 1 week, among 21 (84 %) cases diagnosed as anxiety, STAI were improved yet without significance, 20 (80 %) cases who claimed with fatigue took VAS--f and had a significant improvement, the improvement of VAS--P in 15(60%) cases with pain and related bothers was significant, those 22 (88%) cases claimed they had orthostatic dizziness, but there was no improvement on orthostatic hypotension in all 25 cases. Conclusion STN--DBS can significantly improve motor and some non--motor syndromes in PD patients,but long--term effectiveness yet remain unknown.
