三联疗法联合饮食调护根治Hp感染胃溃疡患者临床观察

Clinical observation on Triple Therapy Combined with Diet Nursing on Helicobacter Pylori Associated Gastric Ulcer

目的探讨三药联合应用在幽门螺杆菌(Hp)相关性胃溃疡治疗中的应用价值,并分析饮食护理对患者饮食保健认知的影响。方法选取我院收治的幽门螺杆菌相关性胃溃疡患者122例,随机分成治疗组、对照组各61例。对照组为常规治疗与护理,治疗组采用三药联合治疗(埃索美拉唑镁肠溶片+阿莫西林+盐酸莫西沙星片)与饮食护理,比较2组治疗效果、炎症因子(IL-6、IL-17)水平、不良反应及患者对饮食保健知识掌握情况。结果治疗组总有效率为96.72%,较对照组的86.89%更高,饮食保健知识掌握优良率为95.08%,高于对照组的77.05%,组间对比差异显著(P<0.05);治疗组不良反应率为8.20%,与对照组的6.56%相较无差异(P>0.05);治疗组治疗后的IL-6、IL-17低于对照组,2组比较有差异(P<0.05)。结论采用埃索美拉唑镁肠溶片三药联合疗法,能进一步改善幽门螺杆菌相关性胃溃疡患者机体炎症状态,提升疗效,并辅以饮食护理,有利于使其掌握更多疾病知识,值得临床推广。

Objective To explore the application value of the combined three drugs in the treatment of helicobacter pylori（Hp） related gastric ulcer,and to analyze the effect of dietary nursing on the cognition of diet health care.Methods 122 cases of patients with helicobacter pylori related gastric ulcer in our hospital were selected for the study and randomly divided into the treatment group and the control group,with 61 cases in each group.The control group was given conventional treatment and nursing,and the treatment group was given the combined three drugs（esomeprazole,amoxicillin and moxifloxacin tablets） and diet nursing.The therapeutic effects,the levels of inflammatory factors（IL-6,IL-17）,adverse reactions,and dietary health care knowledge were compared between the two groups.Results The effective rate in the treatment group was 96.72%,which was higher than 86.89% of the control group.And the excellent and good rate of dietary health knowledge in the treatment group was 95.08%,which was higher than that of the control group with 77.05%（P 〈0.05）.The adverse reaction rate in the treatment group was 8.20%,which was no difference comparing with 6.56% in the control group（P 〈0.05）.The levels of IL-6 and IL-17 after treatment in the treatment group were lower than those in the control group,and there were differences between the two groups（P 〈0.05）.Conclusions The esomeprazole three drugs combined therapy can further improve the body inflammation state and the efficacy in the treatment of patient with helicobacter pylori-associated gastric ulcer.And the supplemented use of diet nursing can help to master more knowledge of the disease.Therefore it is worthy of clinical promotion.
