双金属马钉内固定治疗掌骨骨折疗效分析

Curative effect analysis of metacarpal fracture with internal fixation and double metal horse nail

目的 探讨应用双金属马钉治疗掌骨骨折的远期临床疗效.方法 2008年1月-2016年2月,对16例掌骨骨折患者采用切开复位双金属马钉内固定,术后石膏外固定3~4周,术后视肿胀情况1~2周开始骨折远、近端关节的功能锻炼,术后长期随访,分析临床效果.结果 术后16例获得3~24个月的随访,全部病例在6个月内达到骨性愈合,拆除内固定后受伤手的掌指、指间关节主动活动度均〉60°,未发生内固定松脱、折断以及骨折畸形愈合、延期愈合等情况,手功能按TAM系统评定法评定：优14例,良2例.结论 应用双金属马钉内固定治疗掌骨骨折,能恢复骨折的正常对合并维持骨折的稳定性,远期临床效果良好.

Objective To evaluate the long term clinical efficacy of double metal nail in the treatment of metacarpal fracture. Methods From January 2008 to February 2016, with open reduction and double metal horse nail fixation in 16 patients with metacarpal fracture, postoperative plaster external fixation for 3~4 weeks, according to the swelling, function exercise was implemented at the distal and proximal lateral joint of fracture in 1~2 weeks after operation. Long-term following-up was carried out after operation, clinical effect was analysisd. Results After operation, 16 cases had been followed up for 3~24 months, the longest follow-up was 2 years, all cases within 6 months to achieve bone healing, removal of the internal fixation, the injured hand metacarpophalangeal and interphalangeal joint activity were greater than 60 degrees, no internal fixation loosening, malunion, delayed healing, according to the TAM system evaluation： excellent in 14 cases, good in 2 cases. Conclusion The treatment of metacarpal fracture with double metal nail internal fixation can restore the stability of the bone, and the long-term clinical effect is good.
