Effect of Sodium Aescinate Injection in Treatment of Tibial and Fibular Fractures after Surgery and Influence on Serum BMP,IGF and VEGF Levels

摘要 目的:观察注射用七叶皂苷钠在胫腓骨骨折术后患者中的应用效果及对患者血清骨形态发生蛋白(BMP)、胰岛素样生长因子(IGF)、血管内皮生长因子(VEGF)水平的影响。方法:选择2014年7月~2016年7月我院收治的108例胫腓骨骨折患者,按不同治疗方式分作对照组与研究组,每组54例,两组患者均采用切开复位内固定治疗,对照组术后进行常规处理,研究组在对照组基础上联合注射用七叶皂苷钠治疗。观察并比较两组肿胀、疼痛、瘀斑消失时间,治疗前后血清BMP、IGF、VEGF水平、骨痂生长评分的变化,功能恢复情况及并发症的发生情况。结果:治疗后,研究组肿胀、疼痛、瘀斑消失时间均显著短于对照组(P<0.05)。两组治疗后血清BMP、IGF、VEGF水平、骨痂生长评分均较治疗前显著上升,研究组治疗后以上指标均显著高于对照组(P<0.05)。研究组功能恢复优良率显著高于对照组,且并发症发生率明显低于对照组(P<0.05)。结论:注射用七叶皂苷钠可有效提高胫腓骨骨折术后患者的临床疗效,有利于骨折的愈合,可能与其增加患者血清BMP、IGF、VEGF水平有关。 Objective: To observe the effect of sodium aescinate injection in the treatment patients with tibial and fibular fractures after surgery and the influence on the serum bone morphogenetic protein(BMP), insulin-like growth factor(IGF) and vascular endothelial growth factor(VEGF) levels. Methods: 108 patients with tibiofibula fractures from July 2014 to July 2016 were selected and divided into the control group and the research group according to different treatment methods with 54 cases in each group, the control group was given normal treatment after operation, while the research group was treated with sodium aescinate for injection based on the control group. The swelling, pain and bruising time, changes of serum levels of BMP, IGF, VEGF, callus growth score before and after treatment,function recovery and incidence of complications were compared between two groups. Results: After treatment, the swelling, pain and bruising time of research group were lower than those of the control group(P〈0.05). The serum levels of BMP, IGF and VEGF, and bone scabs of two groups were significantly higher than before, and the research group were significantly higher than those of the control group(P〈0.05). The rate of functional recovery in research group was significantly higher than that of the control group, and the complication rate of research group was significantly lower than that of the control group(P〈0.05). Conclusion: Application of sodium aescinate injection could help fracture healing in the patients with tibial and fibular fractures after surgery, which might be related to the increase of the BMP, IGF and VEGF levels.
