芪棱汤对2型糖尿病并发脑梗死患者内脂素及糖脂代谢的影响

Influence of Qileng Decoction on Serum Visfatin and Glycolipid Metabolism in Patients with Type 2 Diabetes Mellitus Complicated with Cerebral Infarction

目的观察芪棱汤对2型糖尿病并发脑梗死患者血清内脂素及糖脂代谢的影响,并探讨其作用机制。方法 160例患者随机分为芪棱汤组和对照组各80例,对照组采用脑梗死的基础方案治疗,芪棱汤组在对照组基础上加用芪棱汤免煎颗粒治疗,疗程30 d。比较两组治疗前后血清内脂素、空腹血糖(FPG)、餐后2小时血糖(2 h PBG)、糖化血红蛋白(Hb A1c)、三酰甘油(TAG)、总胆固醇(TC)、低密度脂蛋白(LDL)、高密度脂蛋白(HDL)的变化,并评价两组神经功能缺损的临床疗效。结果治疗后,两组的血清内脂素、FPG、2 h PBG、Hb A1c、TAG及LDL水平与治疗前比较均明显降低(均P<0.01),且芪棱汤组治疗后血清内脂素、FPG、2 h PBG、Hb A1c、TAG及LDL水平均低于对照组治疗后的水平(均P<0.05),而两组治疗前后TC、HDL均无变化不大;芪棱汤组神经功能缺损疗效的总有效率为97.50%,高于对照组的93.75%(P<0.05)。结论芪棱汤可明显降低2型糖尿病并发脑梗死患者内脂素及糖脂代谢的水平,提高神经功能缺损的疗效,延缓疾病发展。

Objective： To study the influence of Qileng Decoction on the level of serum visfatin and glycolipid metabolism in patients with type 2 diabetes mellitus complicated with cerebral infarction. Methods： 160 patients with type 2 diabetes mellitus complicated with cerebral infarction were randomized into Qileng Decoction group and the control group. The control group received only basic therapy,while Qileng Decoction group received Qileng Decoction and basic therapy for 30 days. Before and after treatment,serum visfatin,fasting plasma glucose（FPG）,2 h postprandial blood glucose（2 h PBG）,hemoglobin A1 c（Hb A1 c）,triglycerol（TAG）,total cholesterol（TC）,low density lipoprotein（LDL） and high density lipoprotein（HDL） in two groups were compared,and the clinical curative effects in two groups were evaluated with neural function defect scores. Results： After treatment,levels of serum visfatin,FPG,2 h PBG,Hb A1 c,TAG and LDL were significantly reduced,with statistically significant difference compared with those before treatment（P〈0.01）,and levels of serum visfatin,FPG,2 h PBG,Hb A1 c,TAG and LDL in Qileng Decoction group were lower than those in the control group（P〈0.05）. Levels of TC and HDL in two groups did not change significantly. The total curative effective rate of the nerve function defect in Qileng Decoction group was 97.50%,and that in the control group was 93.75%. The difference between two groups was statistically significant（P〈0.05）. Conclusion： Qileng Decoction can obviously reduce the level of visfatin and glycolipid metabolism in type 2 diabetes mellitus complicated with cerebral infarction,improve the curative effect on nerve function defect and postpone the development of the disease.
