强心汤联合增强型体外反搏治疗慢性心力衰竭的临床研究

Clinical Curative Effect of Qiangxin Decoction with Enhanced External Counterpulsation on Patients with Chronic Heart Failure

目的通过对慢性心力衰竭患者应用强心汤联合增强型体外反搏治疗,观察临床疗效和治疗前后血清中血管紧张素Ⅱ(AngⅡ)的变化。方法将符合条件的100例慢性心力衰竭患者随机分为治疗组与对照组各50例,治疗组为西药加中药强心汤、增强型体外反搏治疗,对照组为西药加中药强心汤,观察两组治疗前后的临床疗效及血清中AngⅡ水平、6 min步行距离。结果治疗后治疗组临床疗效和中医证候疗效较对照组效明显改善(P<0.05),6 min步行距离治疗组较对照组增加更加明显(P<0.05);两组血清AngⅡ含量较治疗前均有显著降低(P<0.01),治疗后治疗组血清AngⅡ含量显著低于对照组(P<0.01)。结论强心汤联合增强型体外反搏能更有效地治疗慢性心力衰竭,其作用机制及效应靶点可能与降低血清中AngⅡ水平有关。

Objective： To observe the clinical curative effect of Qiangxin Decoction with enhanced external counterpulsation on patients with chronic heart failure and serum Ang Ⅱ. Methods： 100 patients with chronic heart failure were randomly divided into two groups,50 in the treatment group and 50 in the control group. The treatment group received Western medicine and Qiangxin Decoction with enhanced external counterpulsation,while the control group received Western medicine and Qiangxin Decoction. The clinical efficacy of the two groups before and after treatment,the level of Ang II in the serum and the walking distance of 6 minutes were observed. Results： After treatment,compared with the control group,the curative effect and the curative effect of TCM syndrome of the treatment group were obviously improved（P〈0.05）. The walking distance of 6 minutes in the treatment group increased more obviously than that in the control group（P〈0.05）. The content of serum Ang II in the treatment group and the control group was significantly lower than that before the treatment（P〈0.01）.After treatment,the content of serum Ang II in the treatment group was significantly lower than that of the control group（P〈0.01）. Conclusion： Qiangxin Decoction combined with enhanced external counterpulsation can effectively treat chronic heart failure,whose action mechanism and effect of targets may be related to lowering serum AngⅡ level.
