氯吡格雷治疗不稳定型心绞痛患者血清炎症因子及血管内皮功能的影响

Effects of clopidogrel on serum inflammatory factors and vascular endothelial function in patients with unstable angina pectoris

目的探讨氯吡格雷对不稳定型心绞痛患者血清炎症因子及血管内皮功能的影响。方法 80例不稳定型心绞痛患者随机分为对照组(40例)和观察组(40例),对照组患者给予常规对症治疗,观察组患者在对照组的基础上加用氯吡格雷。连续治疗8周后,比较治疗前后2组血清炎症因子、血管内皮指标变化及治疗后2组临床效果。结果治疗后,2组hs-CRP、IL-6、TNF-α、ET-1水平较组内治疗前均显著降低,且观察组显著低于对照组;2组NO水平均显著高于治疗前,且观察组显著高于对照组;在临床效果方面,观察组总有效率为92.5%,明显优于对照组的72.5%;以上差异均有统计学意义(P<0.05)。结论在常规治疗的基础上加用氯吡格雷治疗不稳定型心绞痛,可有效降低血清炎症因子水平,改善患者血管内皮功能并且具有更好的临床治疗效果,具有临床推广价值。

Objective To investigate the effects of clopidogrel on serum inflammatory factors and vascular endothelial function in patients with unstable angina pectoris. Methods We divided 80 cases of unstable angina pectoris patients randomly into control group（ n = 40） and observation group（ n = 40）; patients in the control group were given conventional therapy,on the basis for the therapy of the control group,the observation group were treated with adding clopidogrel. Both groups were given sustainable treatment for 8 weeks,and the levels of serum inflammatory factors and vascular endothelial were compared before and after the treatment,as well as the clinical effect of the two groups after treatment. Results After treatment,the levels of hs-CRP,IL-6,TNF-α,ET-1 of the two groups were significantly lower than those before treatment,and the observation group levels were significantly lower than those in the control group after treatment; the levels of NO in both groups after treatment were significantly higher than those before treatment,and the observation group level was significantly higher than that of the control group; in the clinical effect,the total effective rate of the observation group was 92. 5%,significantly better than the control group（ 72. 5%）,and the above differences were all statistically significant（ P〈0. 05）. Conclusion Adding clopidogrel based on conventional therapy can effectively reduce the level of serum inflammatory factors,improve the vascular endothelial function in patients with unstable angina pectoris and has a better clinical treatment effect,which has important clinical value.
