凝血功能相关指标、血浆Cys-C水平变化与急性脑梗塞患者病情程度的相关性及临床意义研究

Research on the relevance of coagulation function related indexes and plasma Cys-C levels with severity of acute cerebral infarction and its clinical significance

目的探讨凝血功能指标及血浆Cys-C水平与急性脑梗塞病情相关性。方法选取急性脑梗塞患者187例设为研究组,其中大梗塞灶71例、中梗塞灶64例、小梗死灶52例,轻度55例、中度62例、重度70例,另选取同期健康体检者187例设为对照组。测定D-二聚体、FIB、TT、APTT、PT和Cys-C水平。结果研究组血浆Cys-C、D-二聚体、FIB水平高于对照组,TT、APTT、PT水平低于对照组;不同梗塞病灶患者间血浆Cys-C水平及凝血功能指标存在明显差异;不同病情程度患者间Cys-C水平及凝血功能存在明显差异;Cys-C、D-二聚体、FIB与急性脑梗塞病情呈正相关(P<0.05),TT、APTT、PT与病情呈负相关关系(P<0.05)。结论急性脑梗塞患者血浆Cys-C水平、凝血功能出现异常,且Cys-C及D-二聚体、FIB及TT、APTT、PT水平与病情程度密切相关。

Objective To investigate the relevance of coagulation function indexes and levels of plasma Cys-C with acute cerebral infarction. Methods A total of 187 patients with acute cerebral infarction in our hospital from October 2015 to May 2017 were selected as the study group,including 71 cases with large infarction,64 cases with middle infarction,52 cases with small infarction,55 mild cases,62 moderate cases,and 70 severe cases. Another 187 cases of physical examination were selected as the control group. The levels of D-Dimer,FIB,TT,APTT,PT and Cys-C were determined. Results The levels of plasma Cys-C,D-Dimer and FIB in the study group were higher than those in the control group,while the levels of TT,APTT and PT were lower than those in the control group. There were significant differences in the levels of plasma Cys-C among patients with different infarction lesions. There were significant differences in Cys-C or related indexes of coagulation function among patients with different degrees of severity. The levels of Cys-C,D-Dimer and FIB were positively correlated with the severity of acute cerebral infarction（ P〈0. 05）. The levels of TT,APTT and PT were negatively correlated with the severity of acute cerebral infarction（ P〈0. 05）. Conclusion The levels of plasma Cys-C in patients with acute cerebral infarction and coagulation function were abnormal. The levels of Cys-C,D-Dimer and FIB,TT,APTT and PT were closely correlated with the severity of acute cerebral infarction.
