乳腺肿瘤患者血清肿瘤标志物及T细胞亚群的检测分析

Detection and analysis of serum tumor markers and T lymphocyte subsets in patients with breast neoplasms

目的探讨乳腺肿瘤患者血清肿瘤标志物及T细胞亚群的变化。方法选取乳腺肿瘤患者110例,其中乳腺良性肿瘤组40例,乳腺癌组70例,另选取健康女性50例作为对照组,比较各组血清肿瘤标志物及T细胞亚群水平。结果乳腺癌组的CEA、CA15-3、CA19-9、CYFR21-1水平均高于乳腺良性肿瘤组及对照组,差异有统计学意义(P<0.05),乳腺良性肿瘤组与对照组的血清肿瘤标志物水平差异无统计学意义(P>0.05)。各组4项肿瘤标志物联合检测的阳性率高于各单项指标检测的阳性率,且乳腺癌组显著高于乳腺良性肿瘤组及对照组;乳腺癌组的CD3+、CD4+、CD4+/CD8+低于乳腺良性肿瘤组及对照组,CD8+高于乳腺良性肿瘤组及对照组,差异均有统计学意义(P<0.05)。结论乳腺癌患者存在T细胞亚群紊乱,联合检测CEA、CA15-3、CA19-9、CYFR21-1对筛查乳腺癌、鉴别良恶性病灶具有参考价值。

Objective To investigate the changes of serum tumor markers and T lymphocyte subsets in patients with breast neoplasms. Methods A total of 110 women with breast cancer in our hospital were selected,including 40 cases of benign breast tumors（ benign breast tumor group）,and 70 cases of breast cancer patients（ breast cancer group）. In addition,50 healthy women who underwent physical examination in our hospital were selected as control group. The serum tumor markers and T lymphocyte subsets were compared. Results The levels of CEA,CA15-3,CA19-9 and CYFR21-1 in the breast cancer group were higher than those in the benign breast tumor group and the control group,with the differences statistically significant（ P〈0. 05）. There was no statistical significance in the difference of CEA,CA15-3,CA19-9 and CYFR21-1 levels between the benign breast tumor group and the control group（ P〈0. 05）. The positive rate of combined detection of 4 tumor markers in each group was higher than that of each individual index,and the breast cancer group was significantly higher than that of the benign breast tumor group and the control group; the levels of CD3^＋,CD4^＋and CD4^＋/CD8＋in breast cancer group were lower than those in benign breast tumor group and control group,and CD8＋was higher than that in benign breast tumor group and control group,with the differences statistically significant（ P〈0. 05）. Conclusion There is T lymphocyte subsets disorder,and the combined detection of CEA,CA15-3,CA19-9 and CYFR21-1 has important reference value for screening breast cancer and identifying benign and malignant lesions.
