口腔癌患者血清中白细胞介素-8、Smad 4水平与淋巴结转移度及临床预后的关系 被引量:3

Relationship of serum levels of IL-8 and Smad 4 with lymph node metastasis and clinical prognosis in patients with oral squamous cell carcinoma

摘要 目的探讨口腔癌患者血清中IL-8、Smad 4表达水平与淋巴结转移度及临床预后的关系。方法选取2012年5月-2016年5月在本院行手术治疗的86例口腔癌患者作为研究对象,所有患者均采用酶联免疫吸附法检测癌组织中IL-8、Smad 4表达水平,采用Logistics回归分析探讨癌组织中IL-8、Smad 4表达与患者淋巴结转移度、化疗效果的关系,及与患者临床预后的关系。结果 86例患者中CR 20例、PR 39例、SD 12例、PD 15例,未出现患者死亡;CR、PR组患者IL-8、Smad 4蛋白表达阴性率高于阳性患者,而SD、PD组患者IL-8、Smad 4蛋白表达阴性率低于阳性患者;LNR低水平组患者IL-8、Smad 4阴性表达率明显更高,LNR高水平组IL-8、Smad 4阳性表达和强阳性表达率明显更高。多因素分析结果提示:LNR、Smad 4强阳性、IL-8强阳性是口腔癌患者转移或复发的独立危险因素(P<0.05)。结论 IL-8和Smad 4阳性表达是口腔癌患者术后新辅助化疗效果差,预后差的重要危险因素。 Objective To investigate the relationship of serum IL-8 and Smad 4 levels with lymph node metastasis and clinical prognosis in patients with oral cancer. Methods A total of 86 oral cancer patients in our hospital from May 2012 to May 2016 were selected as the research objects. IL-8,Smad 4 expression levels in all patients were detected by enzyme-linked immunosorbent assay,and Logistics regression analysis was used to explore the relationship of IL-8,Smad 4 expression in cancer tissue with lymph node metastasis,the effect of chemotherapy,and patient's clinical prognosis. Results There were 20 CR patients,39 cases of PR,12 cases of SD and 15 cases of PD in 86 patients,without any death case. The negative rates of IL-8 and Smad 4 protein expression in CR patients and PR patients were higher than those in positive patients. The levels of IL-8,the negative rate of Smad 4 protein expression were lower than that of positive patients. The negative expressions of IL-8 and Smad 4 in LNR low level group were significant. The positive expression and strong positive rate of IL-8 and Smad 4 were significantly higher in LNR high level group. Multivariate analysis indicated that LNR and Smad 4 were strongly positive and IL-8 strongly positive were independent risk factors for metastasis or recurrence of oral cancer( P〈0. 05). Conclusion The positive expression of IL-8 and Smad 4 is an important risk factor for poor prognosis and poor prognosis in patients with oral cancer after neoadjuvant chemotherapy.
