以患儿与家庭为中心的护理干预对白血病患儿心理应激及免疫功能的影响分析

Analysis on the influence of patient and family centered care on psychological stress and immune function in children with leukemia

目的分析以患儿与家庭为中心的护理干预(Patient and family centered care,PFCC)对白血病(leukemia)患儿心理应激及免疫功能的影响。方法 2015年11月至2016年11月我院收治的白血病患儿80例,按照随机、对照、双盲试验原则分为干预组(n=40)和对照组(n=40),对照组患儿行常规护理,干预组患儿行以患儿与家庭为中心的护理干预,采用抑郁量表和焦虑量表评分评估患儿心理状态,流式细胞仪检测外周血T细胞亚群。结果 (1)干预后干预组患儿HAMA、HAMD评分低于对照组(P<0.05);(2)干预后干预组患儿CD8^+低于对照组,CD3^+、CD4^+、CD4^+/CD8^+高于对照组(P<0.05);(3)干预组患儿家属MPOC评分高于对照组(P<0.05)。结论对白血病患儿实施PFCC护理模式,有利于缓解患儿不良心理状况,调节免疫功能,进一步满足患儿家属护理需求,值得临床积极推广和应用。

Objective To analyze the the influence of patient and family centered care on psychological stress and immune function in children with leukemia. Methods Eighty children with leukemia enrolled in our hospital from November 2015 to November 2016 were selected and divided into the observation group（ n = 40） and the control group（ n = 40） according to randomized,controlled and double-blind trial principles. The children in the control group were given routine care,while the children in the observation group were given patient and family centered care. The mental state of the children was evaluated with the depression scale and the anxiety scale,and the flow cytometry was used to detect the T cell subsets in the peripheral blood. Results （1） The scores of HAMA and HAMD in the observation group were lower than those in the control group（ P〈0. 05）;（ 2） the CD8~＋ in the observation group was lower than that in the control group,while the levels of CD3~＋,CD4~＋,and CD4~＋/CD8~ ＋were higher than those in the control group（ P〈0. 05）;（ 3） the scores of MPOC in the observation group were higher than those in the control group（ P〈0. 05）. Conclusion The implementation of PFCC nursing mode for children with leukemia is beneficial to alleviate the unhealthy psychological condition of children,regulate the immune function,and further meet the nursing needs of the children＇s relatives. It is worthy of clinical promotion and application.
