摘要
目的探讨联合TTF-1、CK7、P63、CK5/6、CD56免疫组化在肺癌诊断及组织学分型中的意义。方法选取587例肺癌标本,进行免疫组化染色,所选抗体包括:TTF-1、CK7、P63、CK5/6、CD56。结果TTF-1、CK7在诊断腺癌的灵敏度和特异度分别为87.5%、97.6%和43-3%、89.2%,两者联合诊断肺腺癌的灵敏度和特异度分别为85.6%和99.2%;P63、CK5/6在诊断鳞癌的灵敏度和特异度分别为97.0%、97.6%和91.2%、95.0%,两者联合诊断肺鳞癌的灵敏度和特异度分别为94.6%和100.0%;TTF-1、CD56在诊断肺小细胞神经内分泌癌的灵敏度和特异度分别为90.1%、98.1%和43.3%、98.9%,两者联合诊断肺小细胞神经内分泌癌的灵敏度和特异度分别为88.3%和100.0%。结论联合TTF-1、CK7、P63、CK5/6、CD56免疫组化在确定肺癌组织学分型中意义重大。
Objective To investigate the diagnostic value of TTF-1, CK7, P63, CK5/6, and CD56 in the histological typing of lung cancer. Methods The expressions of TTF-1, CK7, P63, CK5/6, and CD56 were detected by immunohistochemical method in 587 cases of lung cancer. Results The sensitivities and specificities of TTF-1 and CK7 in adenocarcinoma were 87.5% and 97.6% and 43.3% and 89.2%, respectively. The sensitivity and specificity of the combination of TTF-1 and CK7 in adenocarcinoma were 85.6% and 99.2%. The sensitivities and specificities of P63 and CK5/6 in squamous cell carcinoma were 97.0% and 97.6% and 91.2% and 95.0%. The sensitivity and specificity of the combination of P63 and CK5/6 in Squamous cell carcinoma were 94.6% and 100.0%. The sensitivities and specificities of TTF-1 and CD56 in small cell lung cancer were 90.1% and 98.1% and 43.3% and 98.9%, respectively. The sensitivity and specificity of the combination of TTF-1 and CD56 in small cell lung cancer were 88.3% and 100.0%. Conclusions TTF-1, CKT, P63, CK5/6, and CD56 have great significance in the histological typing lung cancer.
