摘要 目的探讨分析自体骨屑联合医用生物材料改进胆脂瘤型中耳炎手术技术的临床效果。方法选取我院2014年11月至2017年11月收治的80例胆脂瘤型中耳炎患者,随机分为研究组(开放式手术联合自体骨屑和纤维蛋白粘合剂填塞)和对照组(开放式手术联合明胶海绵和碘仿纱填塞)各40例。分析比较两组患者的局部渗液量、耳堵塞感及听力恢复情况。结果研究组术后1 d、3 d、7 d的局部渗液量均少于对照组(P<0.05);研究组术后3 d、7 d的耳堵塞感评分均低于对照组(P<0.05);研究组术后3个月、6个月、12个月的听力恢复情况均明显优于对照组(P<0.05)。结论自体骨屑联合医用生物材料改进胆脂瘤型中耳炎手术可改善患者术后渗液和耳堵塞感症状,提高听力恢复效果,具有显著的临床疗效。 Objective To explore and analyze the clinical effect of autologous bone debris combined with medical biomaterials in the improvement of surgical technique of cholesteatoma otitis media. Methods 80 cases of patients with cholesteatoma otitis media admitted to our hospital fi'om November 2014 to November 2017 were selected and randomly divided into study group (open surgery combined with autologous bone debris and fibrin glue stuffing) and control group (open surgery combined with gelatin sponge and iodoform yarn packing), with 40 cases in each group. The local effusion volume, ear blockage and hearing recovery were compared between two groups. Results The local effusion volumes at 1 d, 3 d and 7 d after operation of the study group were less than those of the control group (P〈0.05). The scores of ear blockage at 3 d and 7 d after operation of the study group were lower than those of the control group (P〈0.05). The hearing recovery at 3 months, 6 months and 12 months after operation of the study group was significantly better than that of the control group (P 〈0.05). Conclusions Autologous bone debris combined with medical biomaterial in the improved surgical treatment of cholesteatoma otitis media can improve postoperative effusion and ear blockage symptoms, and improve patients' hearing recovery, with significant curative effect.
