摘要 目的探讨常规MRI与磁敏感加权成像(SWI)对急性脑梗死的诊疗效果。方法选取我院2016年3月至2017年12月收治的160名急性脑梗死患者,分为观察组(79例,SWI检查+常规MRI检查)和对照组(81例,常规MRI检查)。比较两组患者的脑微出血(CMB)检出情况、预后情况。结果观察组的CMB检出率和出血检出面积均明显高于对照组(P<0.05)。常规MRI的敏感度、特异性、阳性预测值均明显低于SWI(P<0.05)。观察组的出血性脑梗死发生率、病死率、NIHSS评分均明显低于对照组,BI明显高于对照组,差异均有统计学意义(P<0.05)。结论 SWI能够准确检测CMB,为急性脑梗死的临床治疗提供依据。 Objective To explore the effect of conventional MRI and susceptibility weighted imaging (SWI) in the diagnosis and treatment of acute cerebral infarction. Methods 160 cases of patients with acute cerebral infarction admitted to our hospital fi'om March 2016 to December 2017 were selected and divided into observation group (79 cases, SWI examination + conventional MRI examination) and control group (81 cases, conventional MRI examination). The detection of cerebral microbleed (CMB) and prognosis were compared between the two groups. Results The of CMB detection rates and detection area of hemorrhage of the observation group were significantly higher than those of the control group (P 〈0.05). The sensitivity, specificity and positive predictive value of conventional MRI were significantly lower than those of SWI (P 〈0.05). The incidence and mortality of hemorrhagic cerebral infarction were significantly lower in the observation group than in the control group (P 〈0.05). The incidence of hemorrhagic infarction, mortality and NIHSS score of the observation group were significantly lower than those of the control group, and the BI was significantly higher than that of the control group, with statistical differences (P〈0.05). Conclusions SWI can accurately detect the CMB, which can provide the basis for clinical treatment of acute cerebral infarction.
