谷胺酰胺对宫颈癌术后盆底野外放疗患者肠道的保护作用观察

Protective Effect of Glutamine on Intestinal Ttract in Patients with Cervical Cancer after Operation

目的探讨谷氨酰胺对宫颈癌术后盆底野外放疗患者肠道的保护作用。方法选取90例宫颈癌术后拟行放疗的患者,随机分为试验组和对照组,各45例。试验组采用谷氨酰胺补充疗法;对照组则采用等量的等渗葡萄糖液静脉滴注。对比2组患者放疗前后的血浆谷氨酰胺浓度、DAO、D-乳糖及外周血淋巴细胞与白细胞计数的变化。结果试验组患者放疗后RTOG急性放射损程度明显低于对照组,差异有统计学意义(P<0.05);放疗后7天检测血浆谷氨酰胺浓度、DAO浓度及D-乳糖浓度比较,试验组明显低于对照组,差异有统计学意义(P<0.05);放疗后7天试验组CD8+及外周白细胞总数明显低于对照组,CD4+相对计数明显高于对照组,差异有统计学意义(P<0.05)。结论宫颈癌术后盆底野外放疗过程中补充谷氨酰胺能够有效减轻患者的胃肠道反应,保护肠道黏膜功能,可以更好地改善患者预后。

Objective To investigate the protective effect of glutamine on intestinal tract in patients with cervical cancer after operation. Methods 90 patients with cervical cancer were randomly divided into the experimental group and the control group. The test group was treated with glutamine supplementation. The control group was treated with the same amount of isotonic glucose solution. The changes of plasma glutamine concentration, DAO, D-lactose and peripheral blood lymphocytes and white blood cell count were compared before and after radiotherapy in the 2 groups. Results The degree of acute radiation loss of RTOG in the experimental group was significantly lower than that of the control group. After 7 days of radiotherapy, plasma gluta- mine concentration,DAO concentration and D-lactose concentration were compared. The total number of CD8 ＋ and peripheral white blood cells in the experimental group was significantly lower than that of the control group 7 days after radiotherapy, and the relative count of CD4 ＋ was significantly higher than that of the control group. The difference was statistically significant P 〈 0.05）. Conclusion Glutamine supplementation can effectively reduce the gastrointestinal tract reaction and protect the intestinal mucosal function during the postoperative radiotherapy of cervical cancer. It can improve the prognosis of the patients.
