社区连续照护模式改善老年慢性病患者不良情绪及生活质量

Improvement Function of Community Continuous Care Model on Negative Emotion and Quality of Life in Elderly Patients with Chronic Diseases

为了探讨社区连续照护模式对老年慢性病患者不良情绪及生活质量的影响,本研究选取96例老年慢性病患者分为对照组和实验组各48例,对照组给予常规护理,实验组在常规护理基础上进行社区连续照护,比较两组患者的不良情绪和生活质量。研究发现社区连续照护模式下患者体现出来的愤怒、抑郁、疲劳、紧张及慌乱五个方面的不良情绪的分值均显著低于常规护理,而精力及与自我相关的情绪评分要显著高于常规护理组,差异有统计学意义（p〈0.05）;此外比较两组患者的生活质量评分,发现躯体健康、心理健康、社会支持利用度、睡眠质量这4个指标均显著高于对照组,差异有统计学意义（p〈0.05）,两组患者中均为睡眠质量的评分最高,躯体健康评分最低。说明对老年慢性病患者的治疗不能仅局限于临床可见的病症,还需关注老年人群的心理和生活质量,提供具有针对性的护理措施,合理的社区连续性照护可以较大程度的减少患者的不良情绪,提高其生活质量,也可以提高社区利用度,缓解社会的医疗支出压力。

The research aimed to investigate the effects of community continuous care model on negative emotion and quality of life in elderly patients with chronic diseases.96 elderly patients with chronic diseases were divided into control group（48）and experimental group（48）,then patients in control group and experiment group both were given routine care.Besides,patients in experimental group were also given community continuous care,negative emotion and quality of life were compared between two groups.The study found that community care mode in patients with continuous reflected five aspects of anger,depression,fatigue,tension and panic had bad mood scores,which were significantly lower than those of the conventional nursing group,while the energy and self related emotion scores were significantly higher than that of the conventional nursing group.The difference was statistically significant between the two groups（p〈0.05）;in addition,the life quality of patients were found,these 4 indicators of physical health,mental health,social support utilization and the sleep quality were higher than the control group,and the differences were statistically significant（p〈0.05）;sleep quality score was highest in both two patients,while physical health scores were both lowest.The treatment on elderly patients with chronic diseases could be not confined to clinical visible lesions,but also need rely on the psychological and quality of life,and targeted nursing measures.Reasonable community continuous care might reduce the negative emotion of patients,improve the quality of life,and also could improve the utilization of the community and alleviate the social medical expenditure.
