央芪膏外敷配合深部热疗对中晚期肝癌患者生存质量、免疫功能及Survivin蛋白的影响

Effect of Yang Qi Ointment with Hyperthermia on the Life Quality and Immune Function and Survivin Protein of Advanced Liver Cancer Patients

目的:观察央芪膏外敷配合深部热疗对中晚期肝癌患者的生存质量、免疫功能及Survivin蛋白的影响。方法:将50例中晚期原发性肝癌患者随机分为对照组和治疗组,对照组给予单纯深部热疗治疗,治疗组在此基础上同时给予央芪膏外敷,连续治疗30d后评价患者的近期疗效、生存质量、免疫功能、不良反应及Survivin蛋白表达水平。结果:观察结束时对照组脱落2例,治疗组脱落3例;治疗后治疗组患者肝功能、卡氏评分、中医临床证候评分均优于对照组,两组比较,差异有统计学意义(P<0.05);治疗组治疗后甲胎蛋白明显低于治疗前,差异有统计学意义(P<0.05);两组近期疗效比较,无显著差异(P>0.05);与治疗前相比,治疗组患者治疗后CD4+、CD4+/CD8+水平显著升高(P<0.05),CD8+细胞数量显著降低,差异有统计学意义(P<0.05);治疗组Survivin蛋白表达水平较治疗前明显下降,差异有统计学意义(P<0.05);两组均未出现明显不良反应。结论:央芪膏外敷配合热疗对中晚期肝癌患者具有较好的治疗作用,能显著提高患者生存质量,控制肿瘤生长,不良反应小,其作用机制可能与调节机体免疫平衡,降低Survivin蛋白表达水平有关。

Objective：To observe the effect of Yang Qi ointment with hyperthermia on the life quality and immune function and Survivin protein of advanced liver cancer patients.Methods：50 patients with advanced primary liver cancer patients were randomly divided into the control group and the treatment group.The control group was treated with hyperthermia treatment,and the treatment group was treated with Yang Qi ointment based on the control group.Evaluated the efficacy,quality of life of patients,immune function,toxicity and the expression level of Survivin protein 30 days after treatment.Results：At the end of observation,2 cases were lost in the control group and 3 cases were lost in the treatment group.After treatment,the liver function,KPS score and TCM clinical syndrome score were better than the control group,and the difference between the two groups was statistically significant（P0.05）.After treatment,AFP in the treatment group was significantly lower than that before treatment（P0.05）.There was no significant difference in the short-term efficacy between the two groups（P0.05）.Compared with those before treatment,the CD4＋and CD4＋/CD8＋levels in the treatment group increased significantly（P0.05）,and the number of CD8＋cells was significantly decreased,the difference was statistically significant（P0.05）.There was no obvious adverse reactions in both groups.Conclusion：Yang Qi ointment combined with hyperthermia has a good therapeutic effect on patients with advanced liver cancer,which can significantly improve the quality of life of patients,control tumor growth,small adverse reactions,its mechanism may be related to the regulation of immune balance,reduction of Survivin protein expression level.
