针刀联合祛湿化瘀解毒定痛汤治疗急性痛风性关节炎

TCM Needle-Knife Treatment Combined with Qushi Huayu Jiedu Dingtong Decoction in Treatment of Acute Gouty Arthritis

目的:探讨针刀联合祛湿化瘀解毒定痛汤治疗急性痛风性关节炎的疗效。方法:选取78例急性痛风性关节炎患者为研究对象,随机分成两组。对照组39例,常规西医方法治疗;观察组39例,针刀联合祛湿化瘀解毒定痛汤治疗。观察治疗前后两组患者相关指标变化情况。结果:对照组临床控制率25.64%、有效率82.05%,观察组临床控制率38.46%、有效率92.31%,两组比较差异有统计学意义(P<0.05)。两组患者治疗后C反应蛋白(C-reactionprotein,CRP)、尿酸(uric acid,UA)、白细胞介素-6(interleukin-6,IL-6)、红细胞沉降率(erythrocyte sedimentation rate,ESR)、丙氨酸氨基转移酶(alanine aminotransferase,ALT)和关节疼痛、关节肿胀、关节活动积分较治疗前明显改善(P<0.05),观察组较对照组改善情况更加显著(P<0.05)。对照组不良反应发生率20.51%,观察组不良反应发生率5.13%,两组比较差异有统计学意义(P<0.05)。结论:针刀联合祛湿化瘀解毒定痛汤能改善急性痛风性关节炎患者临床症状,降低尿酸含量,提高疗效。

Objective： To investigate the clinical treatment efficacy of TCM needle-knife treatment combined with Qushi Huayu Jiedu Dingtong Decoction in treatment of acute gouty arthritis. Method： 78 cases of acute gouty arthritis patients were divided into two groups. There were 39 cases in the control group which were given conventional western medicine treatment,there were 39 cases of the observation group which were given TCM needle-knife treatment combined with Qushi Huayu Jiedu Dingtong Decoction.The relevant indicators of changes before and after treatment were observed. Results： The clinical control rate in control group was25. 64%,the effective rate was 82. 05%. The clinical control rate in observation group was 38. 46% and the effective rate was92. 31%. The difference between the two groups was statistically significant（P〈0. 05）. After treatment,C-reactive protein,UA,IL-6,ESR,Alanine aminotransferase（ALT） and joint pain,joint swelling,activity limit points of both groups were significantly improved compared with that before treatment（P〈0. 05）. That of the observation group was improved more significantly than that of the control group（P〈0. 05）. The incidence of adverse reactions in the control group was 20. 51%,and the incidence of adverse reactions in the observation group was 5. 13%. There was significant difference between the two groups（P〈0. 05）. Conclusion：TCM needle-knife treatment combined with Qushi Huayu Jiedu Dingtong Decoction in treatment of acute gouty arthritis can more significantly improve the clinical symptoms of acute gouty arthritis,reduce uric acid and the curative effect.
