短暂性脑缺血发作危险因素与中医体质的相关性研究

Correlation between Risk Factors of Transient Ischemic Attack and Constitution of Traditional Chinese Medicine

目的探讨短暂性脑缺血发作(TIA)的危险因素及其与中医体质的相关性,为基于体质干预短暂性脑缺血发作提供理论依据。方法采用病例对照研究的方法,选取2014年1月—2016年6月于广东省中医院神经内科住院、急诊留观、门诊就诊诊断为短暂性脑缺血发作的病人共120例为病例组,并选取同时期于我院健康体检中心体检的非脑卒中人群120例为对照组。利用χ2检验和Logistic回归分析方法,筛选短暂性脑缺血发作的危险中医体质和危险因素。结果病例组吸烟、酗酒、高血压病、冠心病、糖尿病、血脂异常、高同型半胱氨酸血症、高尿酸血症、超重/肥胖、痰湿质、气虚质、瘀血质比较均高于对照组(P<0.05);而阳虚质、气郁质、平和质比例均低于对照组(P<0.05)。TIA最主要的体质是痰湿质、气虚质、瘀血质和阴虚质。Logistic多因素分析发现吸烟、超重/肥胖、高血压病、糖尿病、冠心病、血脂异常、睡眠呼吸暂停、高同型半胱氨酸血症、痰湿质、阴虚质、瘀血质是短暂性脑缺血发作的危险因素,而平和质是短暂性脑缺血发作的保护因素。结论短暂性脑缺血发作与中医体质密切相关,可以基于体质防治脑卒中。

Objective To explore the correlation between risk factors of transient ischemic attack（TIA）and traditional Chinese medicine（TCM）constitution.Methods One hundred and twenty patients with TIA from January 2014 to June 2016 were selected as the treat ment group,and 120 patients with non-stroke were selected as the control group at the same time.Chi-square test and Logistic regression analysis were used to analyze the difference of TCM constitution and risk factors between two groups.Results Smoking,alcoholism,history of hypertension,history of coronary heart disease,history of diabetes mellitus,dyslipidemia,hyperuricemia,hyperuricemia,overweight/obesity,sputum in treatment group were significantly higher than that in control group（P 〈0.05）,and the ratio of phlegm-dampness constitution,qi-deficiency constitution,yin-deficiency constitution,and blood stasis constitution were higher,but yang deficiency constitution,yin-yang harmony constitution,and qi-stagnation constitution were lower（P 〈0.05）.According to the multiple conditional logistic regression model,smoking,overweight/obesity,history of hypertension,history of diabetes,history of coronary heart disease,history of dyslipidemia,sleep apnea,hyperhomocysteinemia,phlegm-dampness constitution,yin deficiency constitution,and blood stasis constitution were the risk factors of TIA,while the yin-yang harmony constitution was protecting factor.Conclusion TIA is closely related to TCM constitution.It can be used to prevnt stroke based on TCM constitution.
