前路减压融合内固定与后路单纯减压治疗神经根型颈椎病的临床观察

The Clinical Observation of Anterior Decompression Fusion Internal Fixation and Posterior Simple Decompression in Treatment of RadicularCervical Spondylosis

目的:观察和分析前路减压融合内固定与后路单纯减压在神经根型颈椎病中的临床治疗效果。方法:从2014年12月-2016年12月笔者所在医院收治的所有神经根型颈椎病患者当中选取76例患者作为本次的观察研究对象,将这76例患者根据不同的治疗方法分为观察组和对照组,各38例,观察组采用前路减压融合内固定法进行治疗,对照组采用后路单纯减压法进行治疗,对比两组患者手术时间、住院时间、术中出血量、手术前后VAS评分、JOA评分。结果:观察组患者手术时间、住院时间、术中出血量均显著少于对照组,差异均有统计学意义(P<0.05);术前,两组患者VAS评分和JOA评分比较差异无统计学意义(P>0.05),术后,观察组患者VAS评分和JOA评分均显著优于对照组,差异均有统计学意义(P<0.05)。结论:与后路单纯减压相比,前路减压融合内固定法进行神经根型颈椎病的治疗,能够显著减少手术时间、术中出血量、住院时间,明显改善患者术后VAS评分和JOA评分。

Objective：To observe and analyze the clinical therapeutic effect of anterior decompression fusion internal fixation and posterior simple decompression in the treatment of radicular cervical spondylosis.Method：From December 2014 to December 2016,all the patients with cervical spondylotic radiculopathy were selected as the subject of this study.All patients were divided into observation group and control group according to different treatment methods,38 cases in each group.The observation group was treated with anterior decompression fusion and internal fixation method.The control group was treated with posterior decompression method.The operation time,hospitalization time,intraoperative blood loss,preoperative Postoperative VAS score,JOA score of two groups were compared.Result：The operative time,hospitalization time and intraoperative blood loss were significantly lower in the observation group than in the control group（P〈0.05）.There was no significant difference between the VAS score and the JOA score before surgery（P〈0.05）.After operation,the VAS score and JOA score of the observation group were significantly better than the control group,the difference was statistically significant（P〈0.05）.Conclusion：Compared with posterior simple decompression, anterior decompression fusion internal fixation for the treatment of cervical spondylotic radiculopathy can significantly reduce the operation time,intraoperative blood loss,hospitalization time,significantly improve postoperative VAS score and JOA score.
