111例溃疡性结肠炎患者证型分布及部分发病特点的回顾性分析

The Retrospective Study of 111 Ulcerative Colitis Patients with the Chinese Medicine Syndrome Distribution and the Characteristics the Attacks

目的:了解笔者所在地区溃疡性结肠炎(ulcerative colitis,UC)的中医证型分布情况及不同分期UC中医证型的分布。方法:选取2009年7月-2017年6月徐州医科大学肛肠科住院及门诊的111例诊断为UC并经系统中医药诊疗的病例,按参考文献分为脾肾阳虚证、大肠湿热证、脾胃气虚证、肝郁脾虚证、阴血亏虚证、气滞血瘀证6型。统计各个证型分期病例数、各证型各分期年龄段病例数、各证型病程时间。结果:各证型比例,脾肾阳虚证>大肠湿热证>脾胃气虚证>肝郁脾虚证>阴血亏虚证>气滞血瘀证,其中脾肾阳虚及大肠湿热证患者占比72.92%。UC患者中年段病例数最多,占57.66%。所收集UC病例,病程主要集中在1~3年,其次为6~12个月,且脾肾阳虚证患者病程时间段主要在此2个时间段,大肠湿热证患者主要集中在1~3个月与3~6个月这两个时间段,病程明显较短。结论:本地区UC患者证型分布有地区独特性,以脾肾阳虚及大肠湿热为主,且中年患者居多,与UC流行病学调查情况一致。

Objective：To study on the Chinese medicine（CM） syndrome distribution of ulcerative colitis（UC） and the distribution of CM syndrome types at different staging period.Method：From July 2009 to June 2017,111 UC inpatients and outpatients at the department of Anorectal of Affiliated Hospital of Xuzhou Medicine University were recruited.The syndrome types were divided into 6 types according to literature.They were dampness-heat of Dachang syndrome,Pi-Wei qi deficiency syndrome,Gan depression and Pi deficiency syndrome,Pi-Shen yang deficiency syndrome,blood stasis in the intestinal collaterals syndrome,yin and blood deficiency syndrome.The number of syndromes,the number of cases and the time of each syndrome were statistically analyzed.Result：The UC syndrome types were sequenced from high to low as the Pi-Shen yang deficiency syndrome,dampness-heat of Dachang syndrome,Pi-Wei qi deficiency syndrome,Gan depression and Pi deficiency syndrome,yin and blood deficiency syndrome,blood stasis in the intestinal collaterals syndrome.Most UC inpatients are middleaged（57.66%）.The duration of time of these UC patients are mainly from 1~3 years,then 6 months to 1 year.And the duration of time of Pi-Shen yang deficiency syndrome are longer than the time of dampness-heat of Dachang syndrome.Conclusion：The distribution of UC patients in this region is characterized by regional uniqueness,which is mainly based on spleen and kidney Yang deficiency and large intestine heat, and the majority of middle aged patients,which is consistent with the epidemiological investigation of UC.
