化痰平喘汤联合西药治疗慢性阻塞性肺疾病急性加重期(痰湿阻肺)随机平行对照研究

Randomized Controlled Study of Huatan Pingchuan Decoction combined with Western Medicine in the Treatment of Acute Exacerbation of Chronic Obstructive Pulmonary Disease(Tanshi Zufei)

目的]观察化痰平喘汤联合西药治疗慢性阻塞性肺疾病急性加重期(痰湿阻肺)疗效。[方法]使用随机平行对照方法,将94例患者按就诊顺序号抽签随机分为两组。对照组47例沙丁胺醇1~2片/次,3次/d。氨溴索1~2片/次,3次/d,饭后口服。治疗组47例化痰平喘汤:紫苏子15g,党参20g,半夏10g,蛤蚧15g,黄芪40g,前胡、厚朴、陈皮各15g,甘草、生姜各10g,大枣3枚;西药治疗同对照组。连续治疗10d为1疗程。观测临床症状、肺功能、6min步行距离、不良反应。治疗1疗程(10d),判定疗效。[结果]治疗组痊愈0例,显效18例,有效27例,无效2例,总有效率95.74%;对照组痊愈0例,显效13例,有效25例,无效9例,总有效率80.85%;治疗组疗效优于对照组(P<0.05)。FEV_1、FVC、FEV_1/FVC、6min步行距两组均有改善(P<0.01,P<0.05),治疗组改善优于对照组(P<0.01,P<0.05)。[结论]化痰平喘汤联合西药治疗慢性阻塞性肺疾病急性加重期(痰湿阻肺),疗效满意,无严重不良反应,值得推广。

[Objective] To observe the effect of Huatan Pingchuan decoction combined with western medicine in the treatment of acute exacerbation of chronic obstructive pulmonary disease（Tanshi Zufei）. [Methods] 94 patients were randomly divided into two groups by random parallel control. In the control group, 47 eases of salbutamol were 1-2 tablets/times, and 3 times/d. Ambroxol 1-2 tablets/times, 3 times/d, after meal orally. The treatment group of 47 cases Huatan Pingchuan decoction： Zisuzi 15g, Dangshen 20g, Banxia 10g, Gejie 15g, Huangqi 40g, Qianhu, Houpo, Chenpi each 15g, Gancao, Shengjiang each 10g, Dazao 3 western medicine with the control group. The continuous treatment of 10d was 1 courses. The clinical symptoms, lung function, 6min walking distance and adverse reaction were observed. The 1 course of treatment （10d） was treated and the curative effect was determined. [Results] In the treatment group, 0 cases were cured, 18 cases were markedly effective, 27 cases were effective, 2 cases were invalid, the total effective rate was 95.74%. In the control group, 0 cases were cured, 13 cases were markedly effective, 25 cases were effective, 9 cases were ineffective, the total effective rate was 80.85%, the treatment group was better than the control group （P〈0.05）. The two groups of FEVI, FVC, FEV1/FVC and 6min were improved （P〈0.01, P〈0.05）. The improvement of the treatment group was better than that of the control group （P〈0.01, P〈0.05）. [Conclusion] Huatan Pingehuan decoction combined with western medicine in the treatment of acute exacerbation of chronic obstructive pulmonary disease （Tanshi Zufei） has satisfactory curative effect and no serious adverse reactions. It is worth promoting.
