SYSMEX XE-4000全自动血液分析仪在儿童脑脊液常规检测中的应用

Application of SYSMEX XE-4000 fully automatic hematology analyzer in routine cerebrospinal fluid test in children

目的探讨SYSMEX XE-4000全自动血液分析仪在儿童脑脊液常规检测中的应用。方法选取湖南省儿童医院儿科2016年11月—2017年10月收治的780例进行脑脊液常规检查的患儿作为研究对象,采集患儿脑脊液样本,分别用SYSMEX XE-4000全自动血液分析仪检测以及手工显微镜进行细胞学检测,对比两种方法的检测结果的相符程度。结果当脑脊液中白细胞计数(WBC)<30×10~6/L时,仪器法的检测结果明显高于手工法[×10~6/L:103(50~289)比99(47~285),P<0.05];当WBC≥30×10~6/L时,两者比较差异无统计学意义(P>0.05)。WBC分类计数为(20~100)×10~6/L及≥100×10~6/L时,两种方法对多个核细胞计数(r值分别为0.77和0.97)与单个核细胞计数(r值分别为0.91和0.92)的检测结果均有显著相关性(均P<0.01)。红细胞计数(RBC)>10×10~8/L时,两种方法的检测结果有显著相关性(r值为0.96,P<0.01)。结论 SYSMEX XE-4000全自动血液分析仪对WBC≥30个/μL的检测结果精准,在儿童脑脊液常规检测中可推广使用。

Objective To discuss the application of SYSMEX XE-4000 Fully Automatic Hematology Analyzer for routine cerebrospinal fluid（CSF） test in Children. Methods From November 2016 to October 2017, 780 pediatric patients undergoing routine cerebrospinal fluid test in Hunan Children＇s Hospital were chosen as the study subjects. The CSF samples of the children were collected, then SYSMEX XE-4000 fully automatic blood analyzer and manual microscope were used to make cytological examinations respectively, and the degree of consistency of the results obtained from the two methods was compared. Results When the white blood cell count（WBC） in the CSF was less than 30×10~6/L, the detection result of instrument method was significantly higher than that of manual method [×10~6/L：103（50-289） vs. 99（47-285）, P〈 0.05]; When the WBC was ≥30×10~6/L, there was no statistical significant difference（P 〉0.05） in the comparison between the two methods. When the classification of WBC counts were（20-100）×10~6/L and ≥100×10~6/L, the detection results of instrument and manual methods for poly-nuclear cell count（r values were 0.77 and 0.97） and mononuclear cell count（r values were 0.91 and 0.92） respectively were significantly correlated（all P 〈0.01）. When the red blood cell count（RBC） was 〉10×10~8/L, the results of two methods had significant correlation（r value was 0.96, P〈 0.01）. Conclusion When WBC is ≥30×10~6/L, the detection results obtained by SYSMEX XE-4000 fully automatic hematology analyzer are correct and it can be widely used for routine cerebrospinal fluid test in children.
