单孔与双孔胸腔镜手术切除后纵隔肿瘤的对比研究

A Comparative Study on Two Operations of Posterior Mediastinum Tumor Resection

目的 对比单孔与双孔胸腔镜手术切除后纵隔肿瘤的疗效.方法 2011年3月—2017年2月,苏州大学附属第一医院共收治后纵隔肿瘤54例,按手术方法不同分为两组,A组为单孔胸腔镜手术组,共11例患者;B组为双孔胸腔镜手术组,共43例患者,分析比较两组的手术时间、术中出血量、术后拔胸管时间、术后住院时间和手术后并发症.结果 A组患者平均手术时间较B组短[（59.7±16.3）min vs（91.6±58.4）min];出血量少[（20.2±10.9）mL vs（82.6±40.3）mL],两组间差异有统计学意义（P〈0.05）;A组患者平均胸腔引流时间、手术后住院时间较B组短,分别为[（2.5±1.8）d vs（3.0±1.1）d,（3.6±2.7）d vs（5.5±2.3）d],两组间并发症发生率等数据差异无统计学意义（P〉0.05）.结论 单孔与双孔胸腔镜手术切除后纵隔肿瘤疗效相当.

Objective To compare the therapeutic effect of uniport VATS with that of two-port VATS in posterior mediastinum tumor resection. Methods We retrospectively enrolled 54 patients who underwent operation of posterior mediastinum tumor in The First Affiliated Hospital of Soochow University from March 2011 to February 2017. Among them, 11 patients received uniport VATS （group A） and 43 patients received two-port VATS （group B）. We evaluated the operative time, intraoperative blood loss, duration of drainage, postoperative hospitalization period, and postoperative complications between these two surgical techniques. Results The operative time of group A was shorter than that of group B [（59.7±16.3） min vs （91.6±58.4） min], the blood loss of group A was less than that of group B [（20.2±10.9） mL vs （82.6±40.3） mL], there were all significant difference between two groups （P〈0.05）; The average duration of drainage was shorter than that of group B [（2.5±1.8） d vs （3.0±1.1） d], the mean postoperative hospitalization period was shorter than that of group B [（3.6±2.7） d vs （5.5±2.3） d], while there was no significant difference in the incidence of complications between two groups （P〉0.05）. Conclusion Both operations showed the similar efficacy in the resection of posterior mediastinum tumor.
