中药复方治疗脾虚痰湿型多囊卵巢综合征疗效观察

Therapeutic effects of the compound herbal medicine formula in the treatment of PCOS of spleen deficiency and phlegm damp pattern

目的观察中药复方治疗脾虚痰湿型多囊卵巢综合征(PCOS)的有效性,为中医药治疗PCOS提供客观依据。方法根据鹿特丹标准及中医脾虚痰湿证诊断标准纳入60例脾虚痰湿型PCOS患者,口服同一中药复方治疗3个月经周期。治疗前后,观察纳入患者的性激素、血糖、胰岛素、HOMA-IR、TG、TC、HDL-C、LDL-C、APOA、APOB、LPA、体重、BMI、腰围、臀围、WHR和多毛、痤疮、黑棘皮、溢脂的改善情况以及排卵率。结果中药治疗3个月经周期后,患者LH/FSH、T、AND、SHBG、FINS、TG、HDL-C、体重、BMI、腰围、臀围、溢脂百分率和多毛评分较治疗前明显改善,差异均有统计学意义(P<0.05,P<0.01)。根据血清孕酮的结果判断排卵情况,计算每个月经周期的排卵率分别为25.00%,38.33%,56.67%,总排卵率为40.00%。结论中药复方对脾虚痰湿型PCOS患者的性激素水平、糖脂代谢、人体测量学指标和排卵功能有改善作用。

Objective To observe the effectiveness of compound herbal medicine formula in the treatment of polycystic ovary syndrome（PCOS） differentiated as spleen deficiency and phlegm damp so as to provide the objective evidence for the treatment of PCOS in TCM. Methods According to the Rotterdam consensus criteria and the diagnostic criteria of spleen deficiency and phlegm damp syndrome in TCM,60 patients of PCOS of spleen deficiency and phlegm damp syndrome in TCM were included. The patients were treated with the same compound herbal medicine formula for oral administration for 3 menstrual cycles. Before and after treatment,the sex hormone,blood glucose,insulin,HOM-IR,TG,TC,HDL-C,LDL-C,APOA,APOB,LPA,body weight,BMI,waistline,hipline,WHR,hirsutism,acne,acanthosis-nigricans and seborrhea as well ovulation rate were observed. Results After treated with herbal medicine for 3 menstrual cycles,LH/FSH,T,AND,SHBG,FINS,TG,HDL-C,body weight,BMI,waistline,hipline and seborrhea percentage as well as hirsutism score were all remarkably improved as compared with those before treatment,indicating the significant differences（P〈0. 05,P〈0. 01）. The serum progesterone result was taken to justify the ovulation.The ovulation rates were 25. 00%,38. 33% and 56. 67% separately in each month. The total ovulation rate was 40. 00%. Conclusion The compound herbal medicine formula improves in the sex hormone level,glucolipid metabolism,anthropometric indexes and ovulation function in the patients of PCOS of spleen deficiency and phlegm damp pattern in TCM.
