高分辨率CT对肺小结节及早期肺癌的临床价值分析

Clinical Value of High Resolution CT in Small Pulmonary Nodules and Early Lung Cancer

目的:分析研究高分辨率CT对肺小结节及早期肺癌的临床诊断价值。方法:选取我院2014年5月到2016年12月间收治的经肺穿刺活检术或手术病理学检查确诊肺小结节及早期肺癌疾病的患者88例作为研究对象,将其随机分为两组,每组44例,对照组患者在术前实施常规CT检查,观察组患者则在术前实施高分辨率CT检查,分别对不同方式检查下的肿瘤征象显示情况、图像质量等进行比较,将患者的CT诊断结果与术后病理诊断结果进行对照,计算并比较其各自的诊断符合率。结果:观察组患者的毛刺征、分叶征、棘突征、结节内部钙化征、空泡征、支气管征、胸膜凹陷征、血管集束征等的显示率均显著高于对照组患者,其CT图像优良率分别为95.5%和72.7%,术前诊断与术后病理学诊断结果的符合率分别为95.5%和70.5%,也均有观察组显著优于对照组的情况,比较有统计学差异(P<0.05)。结论:高分辨率CT应用于肺小结节及早期肺癌的临床诊断过程中,可以对肿瘤病灶的内部结构、形态学特征进行更为清晰得显示,图像质量更佳,有利于早期肺癌诊断率准确率的提高,颇具临床应用价值。

Objective：To analyze the clinical value of high resolution CT in the diagnosis of small pulmonary nodules and early lung cancer.Methods：88 patients with small pulmonary nodules and early lung cancer which were treated by lung biopsy or surgical pathology examination confirmed t as the research object in our hospital from May 2014 to December 2016,randomly divided into two groups,44 cases in each group,the control group was treated with routine CT examination before operation,patients in the observation group the implementation of high resolution CT in preoperative examination,were signs of tumor with different ways to check the display,compare the image quality,the results of CT diagnosis of patients with postoperative pathological diagnosis results were compared,calculated and compared their respective diagnostic rate.Results：the observation group of patients with burr sign,lobulation,spine syndrome,thyroid nodules of calcification,vacuole sign,air bronchogram and pleural indentation sign,vascular convergence sign display rate were significantly higher than the control group.The CT images were excellent and good rate of 95.5% and 72.7%,diagnosis and postoperative pathological diagnosis results the coincidence rate were 95.5% and 70.5% before operation,the observation group were significantly better than the control group,were statistically significant difference（ P〈0.05）.Conclusion：the clinical diagnosis of high resolution CT is applied to small pulmonary nodules and early lung cancer,can the internal structure,morphological characteristics of tumor lesions more clearly show that the image quality is better,is conducive to the early diagnosis of lung cancer rate improves the accuracy and has clinical application value.
