256层螺旋CT血管造影在婴幼儿法洛四联症诊断中的应用 被引量:1

摘要 目的:探讨256层螺旋CT血管造影(256-SCTA)诊断婴幼儿法洛四联症(tetralogy of Fallot,TOF)的应用价值。方法:回顾性分析30例婴幼儿TOF患者的256-SCTA诊断结果,并与心脏超声(UCG)进行对比。结果:30例TOF患者经256-SCTA和UCG均能发现和准确诊断TOF的4种主要畸形:室间隔缺损、主动脉骑跨程度、右心室流出道狭窄、右室肥厚。256-SCTA检出体肺侧支动脉血管23例(76.7%),UCG仅检出5例(16.7%),有统计学差异。UCG检查发现右肺动脉缺如1例,经256-SCTA检查证实为肺动脉狭窄。结论:256-SCTA能客观清晰地显示TOF的4种畸形,在心外大血管畸形及体肺侧支动脉诊断方面较UCG有明显的优势,是准确诊断TOF无创性检查的理想选择。 Objective:To evaluate the role of 256-SCTA in the diagnosis of tetralogy of Fallot in infants.Methods:Thirty infants diagnosed as tetralogy of Fallot in which 256-SCTA and ultrasound echocardiography( UCG) were all performed were analysed retrospectively.The results of 256-SCTA and UCG were compared.Results:All of the four anomalies including pulmonary stenosis,ventricular septal defect,overriding of the aorta and hypertrophy of right ventrical can be found and diagnosed accurately by 256-SCTA and UCG.In the 30 patients,major aorta-pulmonary collateral arteries were detected in 23 patients by 256-SCTA and in 5 patients by UCG,which had statistical significance.One patients was diagnosed as absence of right pulmonary by USG and was proved to be right pulmonary stenosis by 256-SCTA.Conclusion:256-SCTA can show all the four anomalies of TOF accurately.Compared with USG,256-SCTA can show extracardiac vascular malformation and major aorta-pulmonary collateral arteries more accurately and should be the ideal noninvasive method in the diagnosis of tetralogy of Fallot.
