摘要
目的:探讨胸腹水中HF-BF%、HF-BF#和CEA的联合检测在良恶性胸腹腔积液鉴别诊断中的价值.方法:收集2014年11月至2016年12月胸腹水标本110例,其中恶性肿瘤53例(恶性组),非恶性肿瘤57例(良性组),进行胸腹水高荧光细胞绝对值(HF-BF#)及比率(HF-BF%)和CEA的测定,并对相关数据进行统计分析.结果:恶性组HF-BF#、HF-BF%和CEA水平均明显高于良性组,差异有统计学意义(P〈0.01);相关性分析显示,胸腹水中HF-BF#和HF-BF%与CEA均未发现明显的关联性;经ROC曲线分析得到HF-BF#、HF-BF%、CEA的单独检测的ROC曲线下面积分别为0.820、0.737、0.933;而两项联合检测(HF-BF#+CEA,HF-BF%+CEA)的ROC曲线下面积均明显高于单独检测.结论:对于良恶性胸腹腔积液的鉴别,高荧光细胞和CEA的联合检测可以提供肿瘤细胞的筛选信息,具有较好的临床应用价值.
Objective: To investigate the value of combined detection of HF-BF%,HF-BF# and CEA in the differential diagnosis of benign and malignant hydrothorax or ascites. Methods: The hydrothorax or ascites of 110 patients admitted to the First People's Hospital of Zhaoqing from December to January 2014-2016 wascollected. The cancer patients (malignant group)in 53 cases, 57 cases of the non-cancer patients (benign group). The CEA,highly fluorescent cells absolute value (HF-BF#)and percentage (HF-BP%)of ascites and pleura/effusions were detected, and related datawere conducted for statistical analysis. Results: The levels ofHF-BF#, HF-Blm/0 and CEA of the malignant group were significantly higher than those of the benign group (P〈0.01). According to the Spearman correlation analysis, there is not obvious relevance between the HF-BF#, HF-BF % and CEA in hydrothorax or ascites. According to 1KOC curve analysis, the of single indicator detection ofHF-BF#, HF-Bt% and CEA area under the curve(AUC)was 0.820, 0.737 and 0.933, The AUC of the two combined detection indicators were significantly higher than those of single indicator detection. Conclusion: The combined detection of high- fluorescent cell and CEA can provide the presence of abnormal cells and has significant clinical attlication value.
