基于DRGs的新疆某三级医院综合服务能力评价体系的研究

目的对新疆某院2012年-2016年DRGs指标进行回顾性分析,评价各专业的发展,确定优势专业和弱势专业,为医院精细化管理提供客观的参考数据。方法利用北京疾病相关诊断分组方案(BJ-DRGs)作为风险调整工具从产能、效率、质量3个维度对医院进行评价,同时结合新疆重点专科评审评价指标,在数据标准化的基础分别从横向、纵向评价的综合得分高低来比较27个临床专科的服务能力。结果 2012年-2016年某院医疗服务总产出、医疗服务范围、医疗技术水平呈现逐年递增趋势,内科系统横向评价的综合得分为14.30,对医院的贡献度为57.36%;外科系统得分为10.63,对医院的贡献度为42.64%,内科系统医疗资源消耗是外科的1.56倍;纵向评价显示74.07%的专业发展势头良好,内科系统发展是外科的1.15倍;医院共有7个优势专业。结论运用DRGs指标从横向与纵向2个维度对医院服务能力进行评价,可以在加强弱势专科发展的同时促进优势专科的形成,进一步提升医院核心竞争力。

Objectives A retrospective analysis of DRGs indicators in a hospital in Xinjiang from 2012 to 2016 was conducted to clarify the comprehensive strength of hospitals, evaluate the development of various professions, and identify the dominant and disadvantaged professions, so as to provide objective reference data for fine management of hospitals. Methods The diagnosis of diseases in Beijing（BJ-DRGs） grouping scheme as a risk adjustment tool from the three dimensions of productivity, efficiency and quality of hospital evaluation, combined with the Xinjiang review key specialty evaluation index is determined, using a special formula based on the standard data on the total score, respectively from the horizontal and vertical evaluation scores compared to 27 A clinical specialist service ability. Results From 2012 to 2016 in a hospital medical service, medical service output range, the level of medical technology gradually increased year by year, the comprehensive score of internal horizontal evaluation system is 14.30, the hospital＇s contribution was 57.36%; the surgical system score of 10.63, the hospital＇s contribution was 42.64%, internal system of medical resource consumption is 1.56 times of surgery the longitudinal evaluation shows that the professional development momentum; 74.07% good medical system development is 1.15 times of surgery; the hospital has a total of 7 professional advantage. Conclusions Using DRGs index to evaluate the hospital service ability from two dimensions,horizontal and vertical, can enhance the development of disadvantaged specialties, promote the formation of dominant specialties, and further enhance the core competitiveness of hospitals.
