针刺骨法配合委中放血治疗腰椎间盘突出症疗效观察

Therapeutic Effects on Lumbar Disc Herniation Treated with Bone-Needling Technique and Bloodletting at Weizhong(BL40)

目的观察针灸刺骨法结合委中放血与口服布洛芬缓释胶囊治疗腰椎间盘突出症的近、远期疗效差异。方法将70例腰椎间盘突出症患者随机分为治疗组和对照组,每组35例。治疗组取穴人中、后溪、肾俞、腰阳关、风市、复溜、束骨,配合委中刺血拔罐,对照组口服布洛芬缓释胶囊,均连续治疗15 d。采用疼痛视觉模拟量表(VAS)、JOA下腰痛评分表作为疗效评定标准,分别在治疗后15 d及治疗结束2个半月后进行评价。结果治疗组总有效率为94.3%(33/35),优于对照组的80%(28/35),两组比较差异有统计学意义(P<0.05);治疗后两组JOA下腰痛评分及VAS疼痛评分较治疗前均有明显改善,差异有统计学意义(P<0.01),且治疗组较对照组改善更明显,两组比较差异有统计学意义(P<0.05)。结论针灸刺骨法结合委中放血治疗腰椎间盘突出症疗效确切,能有效改善患者腰椎间盘突出的临床症状,其疗效优于单纯口服西药治疗,并具有较好的远期疗效。

Objective To observe the differences in the long-term and short-term effects on lumbar disc herniation（ LDH） treated with bone-needling technique and bleeding at Weizhong（ BL40） as compared with oral administration of ibuprofen. Methods A total of 70 patients of LDH were randomized into a treatment group and a control group,35 cases in each one. In the treatment group,acupuncture was used at Renzhong（ GV26）,Houxi（ SI3）,Shenshu（ BL23）,Yaoyangguan（ GV3）,Fengshi（ GB31）,Fuliu（ KI7） and Shugu（ BL65）. Bleeding and cupping method was used at Weizhong（ BL40）. In the control group,ibuprofen was prescribed for oral administration. The treatment was given continuously for 15 days. VAS scores and JOA scores of the lower limbs were taken as the indexes for the assessment of therapeutic effects. The evaluation was done 15 days and 2. 5 months after treatment separately. Results The total effective rate was 94. 3%（ 33/35） in the treatment group,better than 80%（ 28/35） in the control group,indicating the significant difference（ P 0. 05）. After treatment,JOA scores and VAS scores were all remarkably improved as compared with the scores before treatment in the two groups,indicating the significant difference（ P 0. 01）. The improvements in the treatment group were more remarkable than the control group,indicating the significant differences（ P 0. 05）. Conclusion The bone-needling technique and bleeding at Weizhong（ BL40） achieve the definite therapeutic effects on LDH. The clinical symptoms of lumbar intervertebral disc herniation can be effectively improved,more effective than the simple administration of western medicine. Additionally,the long-term effects of this therapy are satisfactory.
