社区护理对脑卒中后肢体功能障碍患者抑郁状态的效果研究

The effect of community nursing on post-stroke depression in patients with physical dysfunction

目的探讨社区护理干预对脑卒中后肢体功能障碍患者抑郁状态的影响。方法选取2016-03—2017-03在郑州大学第一附属医院神经内科住院确诊为脑卒中后抑郁状态的患者共100例,随机分为干预组和对照组各50例,对照组给予家庭自我康复护理,干预组在此基础上由神经内科护理人员建立社区脑卒中患者档案,通过成立"社区脑卒中病友俱乐部",实施健康宣教(肢体功能锻炼、正确生活方式的指导及发放健康宣教手册)和心理干预(开展社区活动及组建"脑卒中之家"微信群)等措施。比较2组患者干预前及干预6个月后的汉密尔顿抑郁量表(Hamilton Depression Scale,HAMD)得分。结果干预后2组HAMD评分差异有统计学意义(P<0.05),干预组干预后HAMD评分明显低于干预前,差异有统计学意义(P<0.05)。结论社区护理可改善脑卒中后肢体功能障碍患者的抑郁状态,使患者正确面对疾病,增强其战胜疾病的信心,提高生活质量。

Objective To explore the effect of community nursing on post-stroke depression in patients with physical dysfunction. Methods 100 cases diagnosed with post-stroke depression in our hospital from March 2016 to March 2017 were randomly divided into intervention group and control group, the control group were given family self rehabilitation nursing, the intervention group were given community nursing, including procedures of the establishment of archives of community stroke pa- tients, establishment of ＂community stroke patients club＂, implementation of health education （including teaching physical exercise and the correct way of life guidance and providing health education manual）, psychological intervention （carrying out community activities and the formation of ＂stroke house＂ WeChat group）. Results After intervention,the difference of HAMD score between the two groups was statistically significant （P〈0.05） ,and the HAMD score of intervention group after intervention was significantly lower than that before intervention, the difference was statistically significant （P〈0. 05）. Conclusion Community nursing can improve the depression status of patients with physical dysfunction after stroke, help patients to face the disease correctly, enhance their confidence to overcome the disease, and improve the quality of life.
