腺样体切除联合置管术治疗小儿难治性分泌性中耳炎的临床观察

The Clinical Observation of Adenoidectomy Combined with Catheter for the Treatment of Intractable Secretory Otitis Media

目的探讨腺样体切除联合置管术治疗小儿难治性分泌性中耳炎的临床效果。方法按照随机数字表法将2016年3月-2017年7月84例小儿难治性分泌性中耳炎患儿分组。常规手术组采用耳内镜下置管手术进行治疗,联合手术组采用腺样体切除联合置管术治疗。比较两组难治性分泌性中耳炎治疗转归率;中耳腔积液消失时间、鼓膜痊愈(穿孔愈合)时间;干预前后患儿听力恢复状况、生活影响因子。结果联合手术组难治性分泌性中耳炎治疗转归率高于常规手术组,P<0.05;联合手术组中耳腔积液消失时间、鼓膜痊愈时间短于常规手术组,P<0.05;干预前两组听力、生活影响因子相近,P>0.05;干预后联合手术组听力恢复状况、生活影响因子优于常规手术组,P<0.05。结论腺样体切除联合置管术治疗小儿难治性分泌性中耳炎的临床效果确切,可有效改善患儿临床症状,消除积液症状,促使听力恢复正常,缩短治疗时间。

The routine surgery group was treated by endoscopic catheter surgery,combined with adenoidectomy combined treatment. Results The treatment of intractable secretory otitis media was higher than that of conventional operation group,P 〈0. 05. In the combined operation group,the time of the effluent of the middle ear cavity effluent,the time of tingling in the ear mucosa,the time of the eardrum recovery,and the pre-onset of hearing recovery were shorter than the normal operation group,P 〈0. 05; The two groups of hearing and life influencing factors were similar in the first two groups,P 〉0. 05; After intervention,the hearing recovery status and life influencing factor of the combined operation group were better than that of the conventional surgery group,P 〈0. 05. Conclusions Adenoidectomy combined tube insertion. we exactly the clinical effect of treatment of children with intractable secretory otitis media,which can effectively improve the clinical symptoms,eliminate effusion and tinnitus symptoms,prompted the hearing back to normal,shorten the treatment time.
