摘要
目的分析双源CT(DSCT)血管成像在主动脉缩窄(COA)合并心血管畸形诊断中的应用价值。方法收集2015年1月至2017年2月吉林油田总医院收治的17例COA患者的临床资料,对本类疾病的DSCT血管成像特点进行回顾性分析,并与手术结果进行对照。结果本组17例患者均经手术证实,术前DSCT血管成像显示5例单纯性COA,12例COA合并心血管畸形。合并心血管畸形的病例包括8例动脉导管未闭、5例肺动脉高压、4例主动脉发育不良、2例迷走左锁骨下动脉、1例大动脉转位,均准确诊断,但是对2例室间隔缺损、1例三尖瓣关闭不全漏诊。结论在COA的影像诊断中,DSCT血管成像可以直观、准确地判断缩窄位置和缩窄程度,可以为临床诊断和制定治疗方案提供可靠的影像学依据。
Objective To analyze the application value of dual-source computerized tomography(DSCT) angiography in the diagnosis of aortic coarctation and cardiovascular deformity. Methods From January 2015 to February 2017, the clinical data of 17 patients with aortic coarctation were collected in Jilin oil field general hospital. DSCT vascular imaging characteristics of this disease were analyzed retrospectively, and compared with surgical results. Results All 17 patients in this group underwent surgery, and the preoperative DSCT angiography showed 5 cases of simple aortic coarctation and 12 cases of aortic coarctation combined with cardiovascular deformity. Combined cardiovascular deformity, including 8 cases of patent ductus arteriosus, 5 cases of pulmonary hypertension, 4 cases of aortic dysplasia, 2 cases of vagus left subclavian artery, 1 case of aorta transposition were diagnosed accurately, but 2 cases of ventricular septal defect, 1 case of tricuspid regurgitation in missed diagnosis. Conclusions In imaging diagnosis of aortic coarctation, DSCT vascular imaging can intuitive, accurately judge the position and degree of narrow, provide reliable imaging basis for clinical diagnosis and treatment.
