Treatment of Mason type Ⅱ radial head fractures with Herbert screws

目的探讨Herbert螺钉治疗MasonⅡ型桡骨头骨折的诊疗方法和临床疗效。方法自2011年3月至2016年5月选择Herberl螺钉治疗MasonⅡ型桡骨头骨折患者16例，同时对并发的肘关节周围软组织损伤进行了相应处理。结果术后16例随访10～16个月，平均12．6个月。骨折均临床愈合，无切口感染等并发症，肘关节功能按Broberg-Morrey评分：优14例，良2例。结论Herbert螺钉内同定是治疗MasonⅡ型桡骨头骨折的有效方法，优点是可以提供早期功能锻炼的固定强度，可跨过“安全区”手术，手术创伤小，对前臂的旋转功能影响小，并避免二次手术的不便和风险，但前提是对肘部软组织及韧带进行良好的综合处理。

Objective To explore the treatment method and clinical efficacy of Herbert screws in the treatment of Mason type Ⅱ radial head fractures. Methods From March 2011 to May 2016, Herbert screws were used to treat 16 patients with Mason type Ⅱ radial head fractures, and simultaneously the soft tissue injuries around the elbow joint were dealt with accordingly. Results Postoperatively all the patients were follow-up for 10 to 16 months with an average of 12.6 months. All the fractures were clinically healed without any complications such as incision infection. Elbow joint function was evaluated by Broberg- Money score. The results were excellent in 14 cases and good in 2 cases. Conclusion Herbert screw internal fixation is an effective method for treatment of Mason type Ⅱ radial head fractures. It can provide stability for early functional exercise and can undergo operations across ＂security zone＂. It also has little effect on the rotation function of the forearm and avoids the inconveniences and risks of secondary surgery on the premise of a good comprehensive treatment of the soft tissue and ligaments around elbow joint.
