微导管成袢技术联合介入栓塞治疗颅内动脉瘤的效果观察

摘要 目的研究观察微导管成袢技术联合介入栓塞治疗颅内动脉瘤的临床综合疗效。方法选取该院2014年1月‐2016年1月收治的70例颅内动脉瘤患者为研究对象,将其随机分为观察组和对照组,每组患者各35例。其中观察组采用微导管成袢技术联合介入栓塞治疗,对照组采用常规开颅术治疗。观察比较两组患者的治疗效果及手术安全性。结果观察组患者术后住院时间、美国国立卫生研究院卒中量表(NIHSS)评分均低于对照组患者(P<0.05);观察组患者日常生活能力量表(ADL)评分高于对照组患者(P<0.05);观察组患者手术并发症发生率为5.71%(2/35),显著低于对照组患者的25.71%(9/35)(P<0.05);18个月后随访,观察组患者生存率为77.14%(27/35),显著高于对照组患者的45.71%(16/35)(P<0.05),提示观察组患者远期疗效优于对照组患者。结论微导管成袢技术联合介入栓塞治疗颅内动脉瘤患者的临床疗效显著,改善患者神经功能,提高患者的日常生活能力,且手术安全性好,有较高的临床应用价值。 [Objective] To investigate the clinical efficacy of microcatheter looping technique combined with interventional embolization in the treatment of intracranial aneurysms. [Methods] Seventy patients with intracranial aneurysms treated in our hospital from January 2014 to January 2016 were selected and randomly divided into the observation group and the control group, with 35 patients in each group. The observation group was treated by microcatheter looping technique combined with interventional embolization, while the control group was treated with conventional craniotomy. The curative effect and operation safety of the two groups were observed and compared. [Results] Postoperative hospital stay was shorter, the National Institutes of Health Stroke Scale (NIHSS) score was lower in the observation group than that in the control group (P 〈0.05); Activities of Daily Living (ADL) Scale score of patients in the observation group was higher than that in the control group (P 〈0.05); the complication rate of patients in the observation group was 5.71% (2/35), which was significantly lower than 25.71% (9/35) in the control group (P 〈0.05); after 18 months of follow-up, the survival rate of patients in the observation group was 77.14% (27/35), which was significantly higher than 45.71% (16/35) in the control group (P 〈0.05), suggesting that the long-term efficacy in the observation group was superior to that in the control group. [Conclusion] Microcatheter looping technique combined with interventional embolization in treating patients with intracranial aneurysms has significant clinical curative effect, which can improve neurological function and activities of daily living of patients, with high operation safety and clinical application value.
