磺胺嘧啶银锌霜联合莫匹罗星在烧伤创面治疗中的疗效观察

Efficacy of silver sulfadiazine zinc cream combined with mupirocin in treatment of burn wounds

目的观察磺胺嘧啶银锌霜联合莫匹罗星治疗烧伤创面的临床效果,探讨两者联合使用治疗烧伤创面的优点及可行性。方法选取2013年8月‐2015年8月该院收治的76例大面积烧伤患者作为该次研究的对象,将患者随机均分为观察组与参照组。观察组采用磺胺嘧啶银锌霜联合莫匹罗星治疗,参照组单纯敷用聚维酮碘纱布治疗,观察并比较两组治疗后的创面愈合时间、细菌清除情况及创面回缩情况等指标。结果治疗后4 d时两组创面回缩率相比差异无统计学意义(t=0.218,P>0.05);治疗后第10、16和22天时,观察组创面回缩率均显著高于参照组(t=6.235、3.547、4.625,均P?<0.05);观察组创伤完全愈合时间明显短于参照组(t=6.370,P<0.05);观察组细菌清除率85.0%,显著高于参照组的47.6%(χ~2=12.136,P<0.05)。观察组治疗10 d后肉芽改善率94.7%,明显高于参照组的65.8%(t=10.100,P<0.05),差异有统计学意义。结论磺胺嘧啶银锌霜联合莫匹罗星治疗烧伤创面,能够有效清除细菌,改善创面情况,促进创面愈合,适合在临床上推广使用。

[Objective] To observe the clinical effect of sulfanilamide pyrimidine silver zinc cream combined with mupirocin ointment in the treatment of burn wounds, and to investigate the merits and feasibility of burn wound treatment. [Methods] Seventy-six cases of large area burn patients admitted to our hospital from August 2013 to August 2015 were selected as the subjects of this study, and the patients were randomly divided into observation group and reference group. The observation group was treated with silver sulfadiazine zinc cream combined with mupirocin, the reference group was treated with povidone iodine soaked gauzes, wound healing time, bacterial clearance, the healing shrinkage index were observed and compared between the two groups after treatment. [Results] At day 4 after treatment, there was no statistically significant difference in wound retraction between the two groups （t =0.218, P 〉0.05）; 10, 16 and 22 days after treatment, the wound contraction rate in the observation group was significantly higher than those in the reference group （t =6.235, 3.547 and 4.625, P 〈0.05）; complete healing time of wound of the observation group was significantly shorter than that of the reference group （t =6.370, P 〈0.05）; bacterial clearance rate of the observation group （85.0%） was significantly higher than that of the reference group （47.6%） （Z2=12.136, P 〈0.05）. Ten days after treatment, the improvement rate of granulation in the observation group was 94.7%, which was significantly higher than 65.8% in the reference group （t =10.100, P 〈0.05）. [Conlusion] Sulfadiazine silver zinc cream combined with mupirocin in treatment of burn wound can effectively remove bacteria and improve the wound, promote wound healing, and is suitable for popularization and application in clinic.
