右美托咪啶预处理对骨科下肢手术患者缺血再灌注致心脏损伤的保护效果 被引量:1

摘要 目的探讨右美托咪啶预处理对骨科下肢手术患者止血带引起缺血再灌注致心脏损伤的保护作用。方法选择该院2015年3月‐2017年3月收治的42例行骨科下肢手术患者为研究对象,随机分为对照组与观察组,每组21例,所有患者均行腰丛-坐骨神经联合阻滞麻醉,观察组患者在上止血带前15 min给予右美托咪啶预处理,对照组在相同时间给予等量生理盐水,于两组患者上止血带前(T1)、松止血带后15 min(T2)、2 h(T3)、6 h(T4)、24 h(T5)时测定冠状动脉流出量(CF)及左心室发展压(LVDP)、左心室内压最大上升/下降速率(±dp/dtmax)。结果两组患者T2、T3、T4、T5时间点的CF、LVDP、±dp/dtmax均有下降,观察组从T3时间点开始,CF、LVDP、±dp/dtmax呈逐渐上升趋势,与对照组相比,差异有统计学意义(P<0.05)。结论右美托咪啶预处理可减少骨科下肢手术使用止血带引起的缺血再灌注心脏损伤,对心脏具有一定保护作用。
