摘要
目的评价全膝关节置换术(TKA)后膝关节屈曲功能与麻醉前、后膝关节活动度(ROM)的相关性。方法选取本院2016年1月至2017年1月因骨性关节炎行膝关节置换的患者108例,由同一外科医师实施的初次TKA。术前测量患者麻醉前、后膝关节ROM,与术后1、6、12个月ROM进行相关性分析。结果麻醉前ROM与术后1、6、12个月ROM的相关系数分别为0.392、0.665、0.708(P<0.001),而麻醉后ROM与术后1、6、12个月ROM的相关系数分别为0.495、0.759、0.793(P<0.001)。术前麻醉前、后ROM与术后1、6、12个月的相关系数差异有统计学意义(P<0.05)。结论同麻醉前ROM比较,麻醉后ROM与患者术后膝关节屈曲功能相关性更大,外科医生在术前规划及与患者进行术后功能恢复沟通中应更加关注麻醉后ROM。
Objective To evaluate the relationship between pre-anesthesia/post-anesthesia range of motion (ROM) and knee flexion function after total knee arthroplasty (TKA). Methods 108 patients with osteoarthritis of knee joint replacement performed by the same surgeon of primary TKA screening from 2016 January to 2017 January in our hospital were enrolled in the study. Correlation analysis were conducted between pre-anesthesia/post-anesthesia ROM and knee flexion function of 1, 6, 12 months after total knee arthroplasty. Results The Correlation coefficient of the Pre-ancsthesia ROM and postoperative ROM are 0. 392,0. 665,0. 708 at 1, 6 and 12 months, while the Correlation coefficient of the Pre-anesthesia ROM and Post-anesthesia ROM of postoperative are 0. 495,0. 759,0. 793 ( P 〈 0. 001 ) , indicating that the correlation difference between pre-anesthesia/post-anesthesia ROM and knee flexion function of 1,6, 12 months after total knee arthroplasty is statistically significant ( P 〈 0. 05 ). Conclusions Post-anesthesia range of motion is a better determinant of postoperative range of motion in patients. This should be taken into consid- eration by surgeons during preoperative planning and in managing patients6xpectations.
