替加环素用于重症感染患者的剂量选择研究现状

Dosage selection of tigecycline in critically ill patients

替加环素是首个甘氨酰四环素类抗生素,因其超广泛的抗菌谱以及对多重耐药致病菌良好的体外抗菌活性,被广泛的应用于重症监护病房严重感染患者中。由于急重症感染患者大多存在基础疾病多、肝肾功能差等情况,加上多数情况已超出替加环素说明书的指导范围,临床医师在选择替加环素治疗剂量时常常无从下手。本文对近几年国内外关于替加环素治疗重症感染及多重耐药菌感染的相关研究报道进行综合整理,对替加环素用于医院获得性肺炎、腹膜透析相关腹膜炎、连续肾脏替代治疗、烧伤科及儿科重症感染时的剂量选择进行阐述。

Tigecycline is the first glycylcycline antibiotic agent, which has a wide antibacterial spectrum and effective antibacterial activity against many multidrug resistant （MDR） bacteria. Now, tigecycline is widely used in critically ill patients in the intensive care unit （ICU）. Patients in the ICU mostly suffer basic diseases with hepatic and renal dysfunction, which increase the difficulty in choosing a suitable dose of figecycline against thr infections. So we reviewed reports both in China and abroad on treating serious infection and MDR infection with tigecycline, and summarized the dosage selection of tigecycline in hospital-acquired pneumonia, peritoneal dialysis-related peritonitis, continuous renal replacement therapy and serious infection in burn patients and children.
