动脉插管灌注化疗联合^125Ⅰ放射性粒子植入治疗胰腺癌 被引量:3

摘要 目的探讨动脉插管灌注化疗联合^(125)Ⅰ放射性粒子植入治疗胰腺癌的临床效果。方法选取50例胰腺癌患者为研究对象,采用随机数字法分为两组,每组25例,对照组受试者采取传统静脉化疗治疗,观察组受试者采取动脉插管灌注化疗联合^(125)Ⅰ放射性粒子植入法治疗。化疗2个疗程或粒子植入1个月后评价两组受试者治疗效果、疼痛缓解情况、生活质量并通过随访调查患者生存时间。结果两组受试者肿瘤治疗客观缓解率和疾病控制率比较差异无统计学意义(P>0.05);观察组受试者疼痛缓解情况、KPS评分、临床受益反应以及生活质量评分均显著优于对照组(P<0.05);两组的中位无进展生存期和中位总生存期的差异无统计学意义(P>0.05)。结论动脉插管灌注化疗联合^(125)Ⅰ放射性粒子植入治疗胰腺癌具有较好的临床效果,可有效缓解患者疼痛情况,提高患者生活质量,延长生存时间,疗效优于传统静脉化疗法。 Objective To investigate the effect of arterial infusion chemotherapy combined with iodine - 125 ( 125 I) seed implantation in treating patients with pancreatic cancer. Methods Fifty patients with pancreatic cancer were di- vided into two groups by using random number method. Traditional venous chemotherapy was adopted in control group (n = 25 ), while arterial infusion chemotherapy combined with 1251 seed implantation was adopted in observation group ( n = 25 ). After 2 courses of chemotherapy or 1 month after the seed implantation, the treatment efficacy, pain relief, quality of life and survival time were evaluated. Results After treatment, no significant difference was observed in the objective re- sponse rate (ORR) or the disease control rate (DCR) between the 2 groups (P 〉 0. 05). Pain relief, KPS score, clinical benefit response, and quality of life in observation group were significantly better than those in control group ( P 〈 0. 05 ). No significant difference was observed in the median progression free survival (PFS) or the median overall survival (OS) between the 2 groups (P 〉 0. 05). Conclusion Arterial infusion chemotherapy combined with 125I seed implantation in treating patients with pancreatic cancer has a better clinical effect than traditional venous chemotherapy. It can effectively alleviate pain, improve quality of life, and prolong survival.
