高尿酸患者血清超敏C-反应蛋白水平与胰岛素抵抗发生相关性分析

Relationship analysis of serum super-sensitive C-reactive protein with insulin resistance in patients with hyperuricemia

目的探讨高尿酸患者血清超敏C-反应蛋白(hs-CRP)水平与胰岛素抵抗的相关性。方法 2015年4月-2017年3月于本院治疗的71位高尿酸血症患者为研究对象,根据有无痛风的临床表现分为A组和B组,同时选取同期在本院进行体检的40例健康者作为对照组,采用超灵敏酶联免疫吸附法对2组研究对象进行检测,观察分析各组的hs-CRP水平、空腹血糖(FPG)、血清空腹胰岛素(Fins),胰岛素抵抗(HOMA-IR)指数和β细胞分泌(HOMA-IS)指数的差异。结果 3组的血尿酸和尿尿酸水平存在显著差异(P均<0.05);A、B 2组的hs-CRP、FPG、Fins、HOMA-IR指数均高于健康对照组,而其HOMA-IS指数低于健康对照组,差异有统计学意义(P均<0.05);3组之间的hs-CRP、FPG、Fins、HOMA-IR指数、HOMA-IS指数两两相比均存在差异,差异有统计学意义(P均<0.05);高尿酸血症患者的血清hs-CRP水平和患者的BMI指数(r=0.53)、血尿酸水平(r=0.62)、FIns(r=0.58)和HOMA-IR指数(r=0.71)呈正相关关系(P均<0.05)。结论对于高尿酸血症患者而言,其血清hs-CRP水平与胰岛素抵抗存在密切相关,可以作为一个监测指标,为临床高尿酸血症的治疗提供参考。

Objective To investigate the correlation between serum hypersensitive C-reactive protein（ hs-CRP） and insulin resistance in patients with hyperuricemia. Methods From April 2015 to March 2017,71 patients with hyperuricemia treated in our hospital were selected as the research objects. According to whether arthrolithiasis clinically occurred or not,all the patients were divided into group A and group B. In the meantime,40 healthy subjects who took physical examination in our hospital were selected as the control group,and the two groups of subjects were tested by ultrasensitive enzyme-linked immunosorbent assay.And the differences of fasting blood glucose（ FPG）,serum fasting insulin（ Fins）,insulin resistance（ HOMA-IR） index andβ-cell secretion（ HOMA-IS） index were observed and analyzed. Results The levels differences of serum uric acid and uric acid among the three groups were statistically significant（ P 〈0. 05）; the levels of hs-CRP,FPG,Fins and HOMA-IR in group A and group B were significantly higher than those in healthy control group,but the levels of HOMA-IS index was lower than that of healthy control group,with the differences statistically significant（ P 〈0. 05）. There was statistical significance on the differences in hs-CRP,FPG,Fins,HOMA-IR and HOMA-IS between each two groups（ P 〈0. 05）; the serum hs-CRP level in patients with hyperuricemia was positively correlated with the BMI index（ r = 0. 53）,serum uric acid level（ r =0. 62）,FIns（ r = 0. 58） and HOMA-IR index（ P 〈0. 05）. Conclusion For patients with hyperuricemia,serum hs-CRP levels are closely related to insulin resistance,which can be used as a monitoring indicator for the treatment of clinical hyperuricemia.
