微创经骨膜外钢板内固定术修复骨盆前环骨折的效果

CLINICAL EFFECT OF MINIMALLY INVASIVE PAROSTEAL PLATE INTERNAL FIXATION IN REPAIRING ANTERIOR PELVIC RING FRACTURES

目的探讨微创经骨膜外钢板内固定术治疗骨盆前环骨折的效果。方法回顾性分析2014年9月—2016年6月于我科施行经骨膜外钢板内固定术的40例骨盆前环骨折病人的临床资料。结果 40例病人施行微创经骨膜外桥接钢板内固定术治疗单侧骨盆前环骨折手术时间为30~40min,出血量50~80mL。病人术中均未发生血管损伤。术后40例病人微创手术切口均一期愈合;26例男性病人均无阴囊肿胀、疼痛;1例病人出现大腿外侧的皮肤感觉异常,3个月后消失;2例病人因并发多发骨折形成深静脉血栓。40例病人术后随访6~12个月,X线片显示内固定牢固,骨折愈合良好。根据Majeed骨盆骨折评分系统评价功能,优34例,良6例。结论对于骨盆前环骨折,微创经骨膜外桥接钢板内固定术是一种相对安全性高、手术操作简便、疗效肯定、并发症少的手术方式。

Objective To investigate the clinical effect of minimally invasive parosteal plate internal fixation in repairing anterior pelvic ring fractures. Methods A retrospective analysis was performed for the clinical data of 40 patients with anterior pelvic ring fractures who underwent parosteal plate internal fixation in our department from September 2014 to June 2016. Results For all 40 patients who underwent minimally invasive parosteal plate internal fixation for unilateral anterior pelvic ring fractures,the time of operation was 30-40 min and the volume of blood loss was 50-80 mL.No patient experienced vascular injury during surgery.All patients achieved primary healing of the incision of minimally invasive surgery.All 26 male patients had no symptoms of scrotal swelling and pain;one patient experienced paresthesia of the skin at the lateral side of the thigh which disappeared after 3 months;two patients experienced deep vein thrombosis due to multiple fractures.All 40 patients were followed up for 6-12 months after surgery,and X-ray showed firm internal fixation and good fracture healing.The Majeed pelvic fracture scoring system was used to evaluate pelvic function,and the result showed excellent function in 34 patients and good function in 6 patients. Conclusion Minimally invasive parosteal plate internal fixation is a safe,simple,and convenient surgical method for anterior pelvic ring fractures and has a marked clinical effect and few complications.
