艾滋病病人抗病毒治疗服药依从性与生存质量的相关性

Correlation between adherence and quality of life among HIV/AIDS patients receiving antiviral treatment

目的探讨接受艾滋病（AIDS）抗病毒治疗病人的服药依从性与生存质量现状以及两者的相关性,为制定护理干预策略提供依据。方法应用自行设计的人口学问卷、WHOQOL-HIV-BRIEF简表、服药依从性量表收集信息,运用SPSS 16.0统计软件进行数据处理。结果共98例AIDS病人。在过去7天内的平均服药依从性为91.56%,其中依从性≥95%的仅65例,占66.33%。生存质量各领域得分与常模相比,除精神/信仰领域外,其他领域得分均较低。此外,生理领域、心理领域、独立性领域、社会关系领域、环境领域、精神/信仰领域与常模相比,差异均有统计学意义（P〈0.05）;年龄、职业状况、经济收入、感染途径与生存质量相关（P〈0.05）。同时,依从性好的病人生存质量较好,与依从性差的病人比较,两者在生理领域、社会关系领域、精神/信仰领域以及总分方面差异具有统计学意义（P〈0.05）。结论 AIDS病人生存质量及抗病毒治疗服药依从性有待于进一步提高;依从性好的病人,其生存质量相对较好。

Objective To explore the correlation between antiretroviral treatment adherence and quality of life,among HIV/AIDS patients,in order to provide evidence for nursing intervention strategies.Methods Self-designed questionnaire,WHOQOL-HIV-BRIEF,and medication compliance scale were used to collect information,and SPSS16.0 was used for the data analysis.Results 98 valid questionnaires were received.Average medication adherence of the patients was 91.56% in the past 7 days,and only 65 cases had good compliance,accounting for 66.33%.Compared with the Chinese norm,quality of life among HIV/AIDS patients had a lower score except for spirituality,and the difference was significant in physiological,psychological,independence,social relations,environmental and spiritual domains（P〈0.05）.Age,occupation,income,infection were relevant with quality of life.Patients with good adherence had better quality of life and those with poor adherence had poor quality of life.There was a significant difference in physical domain,social relationship,spirituality and total scores between good adherence and poor adherence（P〈0.05）;Conclusion Medication adherence and quality of life among AIDS patients need to be further improved.Meanwhile,patients with good compliance have better quality of life.It is suggested that clinicians should take corresponding intervention strategies to effectively improve medication adherence and quality of life.
