PON2与LT在儿童睡眠呼吸暂停综合征中的变化分析

摘要 目的探讨对氧磷酶2(PON2)和白三烯(LT)在儿童睡眠呼吸暂停综合征(OSAS)中表达的相关性及临床生物学意义,为探索OSAS细胞乏氧代谢损伤机制提供分子病理学研究基础。方法选择2015年2月至2016年6月于西安市儿童医院初诊确诊为OSAS且未经治疗的患儿32例,以及同期正常体检的儿童18例,以正常儿童群体为对照,比较PON2和LT在OSAS睡眠状态及清醒状态中的差异。结果与正常对照组相比,OSAS(睡眠状态)组中的轻度组和中重度组分别与OSAS(清醒状态)组的患儿比较,PON2和LT的表达水平差异均有统计学意义(t值分别为1.56、1.60、1.45;1.41、1.58、1.60,均P<0.05)。OSAS(睡眠状态)组整体与OSAS(清醒状态)组的PON2和LT表达水平比较差异均无统计学意义(t值分别为1.79、1.81,均P>0.05)。Spearman相关性分析显示,PON2和LT的表达水平与呼吸暂停低通气指数(AHI)分级呈正相关,r值分别为0.78和0.66,均P<0.05。结论 PON2和LT与OSAS患儿的AHI分级存在关联,并可作为独立因子辅助其诊断及分级。 Objective To investigate the correlation and clinical biological significance of expression levels of paraoxonase 2 (PON2) and leukotriene (LT) in children with obstructive sleep apnea syndrome (OSAS) and to provide molecular pathological basis for exploring the injury mechanism of cell hypoxia metabolic in OSAS. Methods Thirty-two children newly diagnosed with OSAS and untreated in Xi'an Children's Hospital from February 2015 to June 2016 and 18 normal children undergoing physical check at the same time were selected. Normal children were set as controls. Differences in PON2 and LT during sleeping and waking period were compared in OSAS children. Results There were significant differences in expressions of PON2 and LT among normal children and children with light, moderate and severe OSAS (sleeping) and children with OSAS (waking) (t value was 1.56, 1.60, 1.45, 1.41, 1.58 and 1.60, respectively, all P〈0.05). No significant differences in expression levels of PON2 and LT were found between OSAS children (sleeping) and OSAS children (waking) (t value was 1.79 and 1.81, respectively, both P 〉 0.05). Spearman correlation analysis showed that expression levels of PON2 and LT were positively correlated with grade of apnea-hypopnea index (AHI) and r value was 0.78 and 0.66, respectively (both P〈0.05). Conclusion PON2 and LT are correlated with grade of AHI in OSAS children, which can be regarded as independent factor for diagnosis and grading of OSAS.
