益气养阴方对室性早搏患者抑郁及QTc间期的影响

Effects of Yiqi Yangyin Formula on depression and QTc interphase of patients with ventricular premature complexes

目的:使用SAS、SDS量表和24h动态心电图,研究益气养阴方对室性早搏患者抑郁及QTc间期的影响。方法:采用随机对照双盲的临床研究方法,收集2015年5月至2016年5月上海中医药大学附属龙华医院门诊及住院患者62例,试验组31例,口服倍他乐克和益气养阴方,对照组31例,口服倍他乐克和5%益气养阴方模拟剂,均治疗3个月后,检测SAS和SDS量表得分,24h动态心电图记录仪采集的QTc间期。结果:治疗前,两组间比较差异无统计学意义;与治疗前比较,治疗后两组SAS和SDS积分显著降低(P<0.01),试验组QTc显著缩短(P<0.01);治疗后,与对照组比较,试验组SAS、SDS显著降低(P<0.01,P<0.05),QTc缩短(P<0.05)及临床疗效总有效率显著升高(P<0.05)。结论:益气养阴方联合倍他乐克能够降低SAS和SDS量表得分,缓解患者焦虑抑郁情绪,能够缩短患者QTc间期,而缩短动作电位平台期,使心室复极缩短,恢复心室肌电稳定性,减轻心脏自主神经损害程度,一定程度恢复交感神经和迷走神经的协调,提高临床有效率。

Objective： To study the effects of Yiqi Yangyin Formula on depression and QTc interphase of patients with ventricular premature complexes by using self-rating anxiety scale （SAS）, self-rating depression scale （SDS） and 24h ambulatory electrocar-diogram. Methods： A randomized, controlled and double blind clinical study was conducted, 62 cases of outpatient and inpatient from May 2015 to May 2016 in Longhua Hospital, Shanghai University of Traditional Chinese Medicine were collected and randomly divided into experimental group （31 cases） and control group （31 cases）. Patients in the experimental group were treated with Yiqi Yangyin Formula and Betaloc, while patients in the control group were treated with 5%stimulate Yiqi Yangyin Formula and Betaloc, after 3 months of treatment, SAS and SDS scores were documented, and QTc interphase detected by 24h ambulatory electrocar-diogram were analyzed. Results： There was no difference between two groups before treatment. Compared with the pretreatment, SAS and SDS scores were significantly reduced in the experimental group and the control group （P〈0.01）, and the QTc was significantly shortened in the experimental group （P〈0.01）. Compared with the control group after treatment, SAS and SDS scores of the experimental group significantly reduced （P〈0.01, P〈0.05）, QTc was significantly shortened （P〈0.05）, while total effective of the clinical efficacy increased significantly （P〈0.05）. Conclusion： Yiqi Yangyin Formula combined with Betaloc could reduce the scores of SAS and SDS, and relieve anxious and depressive mood of patients, and could shorten the QTc interphase and the action potential plateau, further shorten ventricular bipolar, restore the ventricular electrical stability, reduce the level of cardiac autonomic nerve damage to a certain extent, so that to recover the coordination of sympathetic nerve and vagus nerve and improve clinical efficacy.
