黄连温胆汤对血压合并睡眠障碍患者临床症状及血清5-HT、P物质水平的影响

Effect of Huanglian Wendan Decoction on Clinical Symptoms and Serum Levels of 5-HTand P in Patients with Blood Pressure Combined with Sleep Disorder

目的：观察黄连温胆汤对血压合并睡眠障碍患者临床症状及血清5-HT、P物质的影响。方法：选择2015年1月—2015年8月医院收治的129例血压合并睡眠障碍患者,随机分为中药治疗组、西药治疗组和中西医结合治疗组,每组43例。中药组采用黄连温胆汤治疗,西药组采用降压药物氨氯地平及镇静安眠类药物佐匹克隆片治疗,中西医结合组采用黄连温胆汤联合氨氯地平、佐匹克隆片治疗,3组患者均连续治疗3个月,比较3组患者治疗前、治疗后1个月、2个月、3个月临床症状改善及血清5-HT、P物质、匹兹堡睡眠质量评分的变化情况,收集3组患者治疗过程中药物不良反应并比较。结果：中西医结合治疗组患者治疗1个月后、治疗2个月后、治疗3个月后临床症状率、夜间最低血压、血清P物质、匹兹堡睡眠质量评分均明显低于中药组和西药组（P〈0.05）。3组患者治疗期间药物不良反应比较,西药组明显高于中药组（P〈0.05）。结论：对镇静安眠西药不耐受的血压合并睡眠障碍患者,单独采用黄连温胆汤治疗也可取得和西药治疗类似的临床疗效,黄连温胆汤治疗血压合并睡眠障碍具有较高的临床价值。

Objective： To observe the effect of Huanglian Wendan Decoction on the clinical symptoms and serum 5-HT,P substance in patients with blood pressure combined with sleep disorder. Methods： In January 2015-August 2015 in our hospital,129 cases of blood pressure patients with sleep disorderswere randomly divided into Chinese medicine treatment group,western medicine treatment group and combination of traditional Chinese and Western medicine treatment group,each group 43 cases. The Chinese medicine group was treated with Huanglian Wendan Decoction. The Western medicine group was treated with antihypertensive drug amlodipine and sedative hypnotic drug Zopiclone Tablets. The combination group was treated with Huanglian Wendan Decoction combined with amlodipine and Zopiclone Tablets. Three groups were treated for 3 months. Before and 1,2 and 3 monthsafter treatment,we compared three groups＇ improvement of clinical symptoms and serum 5-HT,substance P,Pittsburgh sleep quality score changes and collected three groups＇ adverse drug reactions. Results： In the combination group 1,2 and 3 months after treatment,the clinical symptomsrate of nocturnal hypotension,serum substance P and Pittsburgh sleep quality scores were significantly lower than those of traditional Chinese medicine and Western medicine groups（ P 〈0. 05）. Serum 5-HT was significantly higher in Chinese medicine group and Western medicine group（ P 〈0. 05）. There was no significant difference between the above indexes in Chinese medicine group and Western medicine group（ P 〈0. 05）. During treatment,adverse drug reactions of western medicine group were significantly higher than those of the traditional Chinese medicine group（ P 〈0. 05）. Western medi-cine group and the combination of traditional Chinese and Western medicine treatment group was not statistically significant（ P 〉0. 05）. Conclusion： The blood pressure of patients with hypertension combined with sleep disorders medicine sedative hypnotic intolerance,alone with Huanglian Wendan Decoction and Western medicine treatment can achieve similar clinical efficacy. There is a clinical value of Huang Lianwen Dan Decoction in the treatment of patients with sleep disorder with high blood pressure.
