睾丸微石症对于成年男性精液质量的影响

Effects of testicular microlithiasis on sperm quality in adult males

目的探究睾丸微石症(Testicular microlithiasis,TM)对成年男性精液质量的影响。方法选取本院2014年至2015年的成年男性患者150例作为研究对象,通过阴囊超声诊断TM,诊断为TM的患者分为经典型睾丸微石症(classic testicular microlithiasis,CTM)和局限型睾丸微石症(limited testicular microlithiasis,LTM)。另选取健康志愿者(无男性生殖系统疾病,仅接受常规检查)90例,作为对照组。收集各组人员的临床资料,并对其精液进行收集和检测。结果 150例中,90例(60%)患者被诊断为CTM,平均年龄为(29.45±3.64)岁,60例(40%)患者被诊断为LTM,平均年龄为(28.54±4.17)岁。两组患者的睾丸体积分别为(14.60±2.13)mL,(16.21±3.47)mL,差异有统计学意义(P<0.05)。CTM、LTM和对照组的精子浓度分别为(38.02±31.56)10~6/mL,(52.32±33.264)10~6/mL和(67.15±36.95)10~6/mL;总活力分别为(46.04±23.68)%,(55.36±24.15)%和(62.089±20.44)%;精子前向运动百分率分别为(35.89±20.19)%,(43.16±21.07)%和(47.12±17.85)%,上述指标CTM组与LTM组比较,差异均有统计学意义(P<0.05);CTM组和LTM组分别与对照组比较差异也有统计学意义(P<0.05)。结论 TM与成年男性精液质量密切相关,且CTM组患者的精液质量显著差于LTM组。

Objective To investigate the effects of testicular microlithiasis（TM） on sperm quality in adult males. Methods Adult male patients with TM in our hospital from November 2014 to October 2015 were diagnosed by scrotal ultrasonography. Patients were divided into classic testicular microlithiasis（CTM） and limited testicular microlithiasis（LTM）. In addition, 90 healthy volunteers（no male reproductive system disease, only routine examination） were selected as control group. The clinical data of patients were reviewed, and their semen samples were collected and analyzed. Results Ninety patients（60%）were diagnosed as CTM,and their average age was（29.45±3.64）years.Sixty patients（40%）were diagnosed as LTM,and their average age was（28.54±4.17）years. The testicular volume of the two groups were（14.60 ±2.13） mL,（16.21 ±3.47） mL, the difference was significant（P 〈0.05） sperm concentration of CTM,LTM,and control groups were（38.02±31.56 million/mL vs 52.32±33.264 million/mL vs 67.15±36.95 million/mL, P 〈0.05）,（46.04±23.68% vs 55.36±24.15% vs 62.089±20.44%, P 〈0.05）,and（35.89±20.19% vs 43.16±21.07% vs 47.12±17.85%, P 〈0.05）,respectively. Conclusion TM may be associated with sperm quality in adult males and the sperm quelity of CTM was poorer than that in LTM.
